This thread was requested by Natashina. I am copying over a list of known bugs and issues from the old BSN forums. The good news is that someone (or some people) managed to archive a lot of this information on isitaboutmycube.com. I am merely compiling and copying that information here to avoid extra searching for it. It does not hurt to have it in more than one place.
Natashina started a bug list thread for the rest of the trilogy (Mass Effect 1, Mass Effect 2, Mass Effect 3 single player) here. I volunteered to compile a list of the ME3M bugs, and Natashina suggested I create a new thread in the multiplayer forums here.
Where possible, I have retained or updated links, but I had to remove a lot of links to the old BSN site. I have done my best to give credit to the people who originally posted the information.

Known Bugs and Glitches Thread, compiled by Sunspot01

Character/power/weapon specific bugs:
1. Asari Huntress Infiltrator - Tactical cloak gives bonus to powers only, not to weapons. Weekly balance has boosted the power to this class ONLY to compensate.
2. Awakened Collector - Ascension mode doesn't display glow animation. More likely to happen off-host. Only visual.
3. Awakened Collector - Dark Sphere fails to launch after pressing button. You'll mash the button, go through the animation several times, but no sphere will launch. Generally occurs off-host when you have not detonated the sphere. The game however thinks you have detonated the sphere and you are waiting for the cooldown to be over (without any visual cue).
4. Batarian Brawler Vanguard - Human male vanguard voices are used in Biotic Charge and injury sound effects.
5. Drell characters - unable to use Collector Sniper Rifle and Particle Rifle. When using these rifles, your aim will be re-calibrated to a lower height (for example, Volus) and aim down at the floor. Cover firing can help avoid the glitch. Drell Video.
6. Geth Juggernaut - Hex Shield does not block Praetorian lasers, is iffy with Ravager shots, and does not block grenades/scion grenades. Mainly due to Hex Shield not blocking splash damage, which all of these do.
7. Turian Characters - Stability evolutions for Armiger Legion and Turian Veteran do not work.
8. N7 Paladin Sentinel - Shield Mastery - Cryo Shield evolution doesn't weaken armor.
9. N7 Paladin Sentinel - Snap Freeze - Tech Combo does no increased damage. Snap Freeze also primes protected enemies (shield/barrier/armor)
10. N7 Paladin Sentinel - Paladin Snap Freeze 6b Tech combos do no increased damage. However, 4b reach gives 2 times boosted Cryo Explosion damage if Snap Freeze is used as a primer.
11. N7 Paladin Sentinel - Energy Drain will sometimes not refill shield when used off-host.
12. Annihilation Field - Will sometimes become reversed (prime explosions while off). Example: On, you have movement speed increase but no damage, priming, or shield leeching. Off, no speed increase but all other elements are active.
13. Biotic Charge - occasionally will not charge enemy
14. Blade Armor - Damage Returned evolution doesn't work on Gold and Platinum difficulties.
15. Electric Slash - Detonate evolution doesn't work.
16. Havoc Strike - occasionally will not charge enemy
17. Shadow Strike - unable to use on Atlas after latest patch
18. Shield Boost - tendency to freeze all other players controls/powers (Hard Melee regains control)
19. Shockwave - Radius evolution glitched. Distance appears to be bugged as well, though may be due to latency issues.
20. Sticky Grenades - Ammo powers apply to these. Also, if a sniper rifle is equipped while using grenades, it will have "hip fire" damage reductions if not used while scoped.
21. Tactical Scan - Headshot bonus evolution isn't applied.
22. Collector Sniper Rifle - will sometimes not reload to full capacity. Sometimes stuck like this through the whole match.
23. Prothean Particle Rifle - will sometimes not reload to full capacity. Sometimes stuck like this through the whole match.
24. Geth Plasma SMG - When used with beam-type weapons, it appears to sound like it is firing at full-speed when fired initially, rather than having a warmup.
25. Hurricane SMG - If one bullet is left, it will not fire. Will require a manual reload.
26. Heavy mods for weapons - Weight penalty is not applied to non-DLC weapons, and when Ultra Light Materials are also not used.
27. Snap Freeze (N7 Paladin Sentinel/Alliance Infiltration Unit) - Fire explosions and tech bursts that are detonated by Snap Freeze center on the Paladin/AIU, not the enemy
28. Due to lag/latency issues, sometimes guns gain or lose shots, such as a 3-shot Raider or the ammo counter reaching negative digits (such as -2).
29. Kishock Sniper Rifle - When zoomed in while in cover, it may not automatically zoom back out when you let go of the zoom button. You must break away from cover to return the view back to normal.
Game-breaking bugs (in some cases ban-able offenses):
1. Escort Drone will disappear (frequently occurs around stairwells). Unknown the amount of time to fix while playing; most players have the host drop to have the wave restart.
2. Gun will disappear from hands, but able to still shoot (visuals effected only)
3. Unable to take cover (Map sync/lag issue)
4. Unable to grab enemy (Map sync/lag issue) - this can cause the enemy to become invincible in some cases. Users report Incinerate, Warp, or other "lasting damage" powers as being the only thing to affect them.
5. Unable to get ammo, enemies take no damage, not shown on map to co-players (Map sync/lag issue)
6. Unable to throw grenade - animation is shown but no explosion. Sometimes grenades are used. (Map sync/lag issue)
7. Unable to shoot/shots show no damage - Hard melee can sometimes break out of this glitch.
8. Unable to see teammates/enemies
9. Missiles going through enemies (map sync/lag issue)
10. Missile Glitchers - Infinite missiles
11. Money Glitchers - hundreds, sometimes millions of credits after a single match
12. Characters (Particularly the Retaliation DLC) have extra points when they level up, allowing users to max all 5 powers)
13. No credit after game is completed, despite being present in credit waves.
14. Sometimes hack objectives do not end immediately after the hack progress bar is filled.
Objective bugs and glitches (non-game breaking):
1. Sometimes the extraction timer is invisible at the end of the game.
2. (PS3 only.) Occasionally you will need to assassinate 5 targets instead of 4. Lag related?
Enemy-specific bugs:
1. Collector - Praetorian lasers go through corners and cover, and can be fired at you when the creatured back is towards you (aka butt lasers)
2. Collector - Praetorian has very large radius for sync kills (aka Magnet grab)
3. Collector - On Hazard and Earth Maps, on Gold and Platinum, on PS3, game is prone to hard-freeze, which only a complete PS3 power cycle can fix.
4. Cerberus - Atlas take more damage than usual with piercing weapons
5. Cerberus - Phantom Damage Reductions only work on host.
6. Geth - Rocket Troopers shooting two/three rockets at once
7. Geth - Hunters shooting while meleeing, staggered, on the floor, lifted, or even stasised. Sometimes even after being killed.
8. ALL - Scions, Primes, Hunters, and Ravengers can sometimes shoot through the N7 Paladin Sentinels deployed shield.
Map-based issues:
1. Reactor (Hazard) - Enemies are able to walk through the closed doors.
2. Rio - Dying on an outer railing will cause you to respawn at the Extraction Zone.
3. Rio - Biotic Charge/Havoc Strike from the center of the map, to another area, and you will sometimes be teleported back to the center.
4. Firebase White (Hazard) - Don't play as a Krogan and go into Rage. You will be virtually unable to see anything when you are outside. Not a glitch per say, but a very annoying oversight in this map's design.
5. Other modes like Turian Stim Packs, Geth Hunter Mode, and Vorcha Bloodlust are difficult but playable.
6. Dagger (Hazard) - It gives half as much ammo/grenades as other maps.
7. Condor - Crates don't accurately refill their grenades/ammo
8. Condor - Prone to invisible enemy issues

The infamous Missile Glitch:
The reason why this actually happens is because projectile weapons and the projectiles they fire are actually two different things according to the game. When switching around, you can cause it so that the game thinks that your Striker's projectiles are actually Cobra Missiles.
A fix for this possibly would have been to get the client to check the projectile with each shot and compare it to the weapon being used. It would potentially mean a lot of checks per second, but since it's purely client side even a toaster of a computer could handle it by today's standards.
Asari Huntress Tactical Cloak:
This one didn't increase weapon damage, and that's because the code for it is simply just missing. It's been ripped out, and it needed to be removed in about 3 different places which makes me think it was intentionally done. They were probably toying with the idea of making her cloak a pure power damage one and forgot to change it back.
N7 Typhoon:
So this gun is supposed to increase damage reduction while in cover, only it doesn't. One of the BioWare posters actually made a post saying they tested it and confirmed that the DR was being applied to your character.
and funnily enough they aren't wrong, it was being applied. It was just being removed instantly because of a math error in the code. If I remember the numbers right, the DR required less than 50% DR to be applied but at any point if you weren't above 90% DR then it would be removed.
Which meant it was mathematically impossible to actually get and keep the DR from this gun, even though it was being applied for a split second.
Shockwave on PC:
I think most of us by now know that you don't take radius increase on PC at rank 4 because it causes Shockwave to stop working.
Despite being a sphere in visual, Shockwave actually uses a cone for hit detection so this evolution increases the angle of that cone. Only problem it, it increases the angle of the cone more and more every time you cast Shockwave so eventually the cone folds back in on itself and stops working. Since the math uses cosine at some point, you can actually get it working again by spamming Shockwave and it'll keep working and stop working periodically.
