2)Why Red lyrium popped up? I'll grant that they really ought to have looked into the other three, but how were they to know this one was Blight-related?
They actually were investigating this....Nathaniel and his team (which may include sibling Hawke) were trying to get to the old thaig when rescued by Hawke. Which makes me wonder, if sibling is a Warden, why did Nate/other Wardens need info from Bartrand? Why not get the info from Bethany/Carver on how to get there?
As for why the HoF didn't die killing the Archdemon, in the epilogue talk with Alistair he mentions the wardens already asking questions about it and says it will be pretty easy for him to play dumb. If Alistair, HoF, or Morrigan don't talk, there's not much the rest of wardens can do about it.

The Wardens knew Fiona was cured by the Architect, but without the Architect around to ask (or study), they couldn't work out how he did it, nor how to replicate it. Fiona mentions in DAI that the Warden did try to put her through the Joining again (seemingly several times), but it consistently failed to work, so it became impossible for her to remain with the Wardens.
Without cutting Fiona open for answers (I doubt they'd learn much even if they had, given the Chantry's taboo limiting the study of anatomy), I think the Wardens investigated as much as they conceivably could, given the circumstances.

As I understand it, they know the Architect sped up the Taint inside her, and that Duncan had cured her using a charm he'd stolen from one of the Architect's minions that had protected him from the same process. If nothing else it might have been worth keeping that charm to see if it cured natural Callings. (Well, assuming they hadn't tried that.) But it might have been worth keeping Fiona around anyway, and sending her into the Deep Roads for stealth missions (which the Wardens usually can't do because darkspawn can sense them.) Especially if it turns out that she can't be Tainted at all, though that sounds like a dangerous thing to test.
