True. It's not just about the paragon decisions, though. The fact of matter is that you are never in control of all the facts. A seemingly safe decision might still lead to an unwelcome result.
You choose to go after the terrorist, but as his heart stops a contingency kicks and and blows up the entire building, or something similar with a partner you weren't aware of. Or the faction of terrorists becomes more aggressive than it otherwise would have, like cornered rat they have nothing to loose anymore.
Or take the Rachni Queen. Given that one rachni queen did survive there must be a non-zero chance for any Rachni Queen to survive, however however unlikely. Given a sufficiently large number of Rachni Queens in before the war, there is likely another survivor. Maybe you kill the Rachni Queen and miss the only chance for peace you ever had. Maybe the game is rigged and you loose either way.
In general, the renegade option should be the "safer" one, of course, as you do take negative effects into account to begin with and be considerably less likely to bite you in the ass. I am just saying: Without perfect knowledge there are no guarantees. The real downside is that you will also make you more enemies and less allies. Both personally and as representative of your faction.
There were some choices that were interesting in that regard in Andromeda. You can save the Krogan or the Salarians, but no playing the hero this time, you have to sacrifice one. A choice where it turned out that I made a bad decision in my first game was letting the technician die to acquire the AI. Turns out that saving the technician by blasting the suicidal AI would have been better. Suppose my ego got the better of me, SAM did tell me of all its lies and I did not want it to get through with it. But that thing was suicidal anyway. So by blasting it you miss the chance to help the Angarra recover their history, but in principle everyone is happy. Well... happy dead in the case of the AI. There were many such choices which were not as clearly good or evil. It remains to be seen what comes out of it in the long run, but if there was a successor to Andromeda, it would have consequences. That design is in principle more interesting, even though it takes another game to actually play out.
