Worst game I've ever played would be Mario's Time Machine for the SNES. It shouldn't even be called a game. It's a fill-in-the-blank history test. Oh, there's a tiny bit of surfing in between test pages, because of course there is, but that's it. Okay, yes, it's 'edutainment'. But they forgot the "-tainment". As is usually the case, I suppose. But then again, at least Bronkie, Health Hero - which is a game about an asthmatic dinosaur that helps teach children how to manage their asthma - was a semi-decent platformer.
Semi-decent may be pushing it, but it was something.
...No, I was not a child with asthma, just a bored adult.
Lately? I don't know. I didn't care for Limbo, which is not a fault of the game, just the fact that I don't much care for puzzle platformers. But it was free.
Somewhat related, I quite liked one of the so-called 'worst' games for the NES - Deadly Towers. I rented it dozens of times.

Of the last few years, the only genuine letdown I can think of is Pillars of Eternity. I really had my nostalgia goggles on for that one. The screenshots and brief gameplay videos looked great, I backed it on Kickstarter, but when actually sat down to play it... *thud*. I thought many of the regions were well done, but unfortunately I didn't find much of the lore (or the characters) particularly interesting, so it was a slog to get through. In the end it wound up a completely forgettable game. I literally can't remember the name of a single character in that game... including my own.

I have another one: Might and Magic 7 - For Blood and Honour - you starts out in fantasy world and all is good but in the end you get to fight aliens on spaceships with magic, swords and blaster pistols.
WTF? Once I got the fucking pistols all the gamplay went downhill really fast. People rant about ME trilogy ending? Ye, play this and see how bad things can go.
Arcomage minigame was quite good though.

Recently I'd say Sword Coast Legend. The story is not that bad and it could have provided a decent experience, but the way they screwed up the D&D 5th edition rules, the awfully messed ability/spell system and the insane/idiotic amount of loot really broke my immersion. Luckily it's not very long, therefore I managed to finish it somehow.
Another recent "not-so-good" experience has been Beyond Divinity. The gameplay imho is awful. Whoever thought that playing an action RPG were you have to control two separate characters at the same time was going to be fun must have been mad or under heavy drugs. And the Battlefields are just horrible. Thanks god they were totally optional and I could avoid them. First time in my life that I totally skipped an entire part of a game in my first playtrough.
Worst game of my life.. I don't know.. probably something that I played on my old 16bit consoles when I was young!

My least favourite game ever? Probably Need for Speed Rivals. There are so many bugs, and the police car AI is broken and the driving mechanics are borderline insane.

I'm usually careful when I pick games, so I can't say I've played *truly* bad ones. I guess the worst one I can remember off the top of my head was Lord Of The Rings: The Fellowship of The Ring (2002). It had excellent voice acting (Tom Kane as Gandalf, Jennifer Hale as Galadriel and Lobelia Sackville-Baggins among others), but the game itself... meh, nothing to write home about.

I once made the huge mistake of buying the Green Lantern game to PS3... I started playing it with a friend, but I think you have to be at least a bit drunk to find any enjoyment in it. So damn repetitive!
Bound by Flame is also a game I have been hugely disappointed by, but then again I was stupidly enough expecting BioWare standards. I think the worst part I was promised character creation, and technically I got it, but damn it was truly bad - 5 different faces and hairstyles and some skintones. Also, you can name your character whatever you like, but people in the game are still going to call you by the default (male) name. And in the cut-scenes your character will always be vulgar no matter how you have role-played them seconds earlier. :lol
Doubt either of these are the worst games I ever played, but I imagine whatever game I played actually was worst have been thoroughly erased from my mental hard drive.

Lately? I can't really think of any. I also don't buy a lot of games new, so that probably helps.
My list for the worst of all time thus far:
--Superman 64. I know, stereotypical answer. I actually played that piece of crap for about 2 hours. Stupid rings. The only reason why the controller didn't go flying was because it wasn't my console.
--Final Fantasy II. Not the SNES version, the one that became known later as FFIV. This game sucked. It was bleak, depressing and a slog. Out of the 5 different characters that come into the 4th slot, 4 of them die in the course of the game. The only redeeming factor was the main villain. Bonus points for the extra dungeon being a boring and depressing rehash of the final dungeon in the main game.
--Final Fantasy XIII. It was one of the least engaging games I've ever played. I'm fine with linear game design, but that was overkill. Combined with the "fun" of scouting the codex just to figure out the world and I played that for 3 hours. I was so digusted that I never bothered with XIII-2. It didn't help that I found all of the characters either 2D, dull, uninspired or all of the above. Or flat out annoying.
--Duke Nukem Forever. Really, do I have to say anything else here?

I have to go back to my child hood for these games...... the majority of the worst games I have ever played were mainly NES related. If you were a kid when the NES was the main console out there....then maybe you, like me, would rent games...That is what my parents would let me do every weekend....fridays after school, I would go to the video store that was 2 blocks away and I would rent 2 games to play for the weekend...as soon as I got home, started playing right away and it was the worst feeling when you realized that 1 or both the games were horrible and you wouldnt play 1 or both past friday afternoon /night.......here are a few names
X-Men, Silver Surfer, Battletoads ( if you could make it past that racing level, you were considered a god ), Ninja Turtles ( the 1st one cuz of that damn dam ) Ghosts N Goblins ( was better on the arcade version ) Top Gun ...I have so many more....at least there were saturday morning cartoons to look forward to.

I'm very picky and rarely end up buying stuff I don't like. I watch/read reviews a lot. So I don't remember a really awful game purchase in the past 15 years.
However, the worst one was without doubt Prince of Persia: Forgotten Sands... I loved the first cell shading remake on the 360. But this one was nothing like it. Super dull. No humor. And a game breaking save bug. So on top of it not being very fun I had to replay an hour or two from a backup autosave... only to be rewarded with a short piss poor ending sequence. Ugh.
The only game on the 360 I never finished was Tomb Raider Underworld. My first game of the series. So boring. It wasn't bad, just not my thing. Kept messing up the jumps and dying. No other character around. So you climb through the jungle in silence. Bioware games were such a revelation. Banter FTW!

Of all time, that'd probably go to Spore. A Great idea for a game with very poor execution in its actual gameplay, most of which ends up inferior to the types of genres they're imitating. Recently? It's a tie between Mirror's Edge Catalyst and TMNT: Turtles in Manhattan. I personally felt the original worked better as an action-focused race from A to B with multiple ways to pull it off. Going Open World didn't do particularly much for the formula, and the story's very meh until the last 20 minutes. TMNT had great boss battles, can't say much else positive on the rest. A highly acclaimed game that frustrated me to no end: Ultimately my problems with the game were of the "It's not my kind of game" variety. Classical adventuring puzzles are at some of their most challenging in Grim, and that sort of gameplay's often beyond my brainpower. Me playing Fandango would be like Sylvius the Mad playing Dark Souls. What could possibly go wr- Even those who love Dark Souls have to admit the original's got serious problems. The Anor Londo Archers are stil the biggest derp in enemy placement history, the curse removal is a big middle finger to the uninitiated, and I could probably list 100 other nitpicks. It's a toughie. ¯\_(ツ)_/¯ You disappoint me.

Kinda hard since I've seen my good share of shit games over the years and I mean shit games, not games I just disliked. Maybe go with DMC2 since it's generally held as the gold standard of how you DON'T do a sequel and it's so bad that 3 and 4 where prequels.

Mass Effect Sequels - Not the worst made games but definitely the ones I've dreaded the most, the jarring self aggrandizing dialogue and characters that would become the foundation of every BioWare game since, miniscule "RPG" with dull average chest high Wall shooting Gallery section. I've had more fun with Binary Domain, a sci if shooter with more engaging enemies, characters and overall story.
Silent Hill Homecoming - What an unspeakably pile of crap
Final Fantasy XIII - 30 hours of tutorial, spent in the company of Hope, as in I hope we can get the clothes back inside quick before any Snow or Lightning happens. The story isn't interested in telling me what it is on about with all its L'Cie and strange characters whose motivations and pretty much any relevant exposition has been relegated to an optional pamplet outside of the actual narrative. The only thing decent about this corridor simulation was the music. That was great.

That's the problem with Dragon Age it focuses on more story less gameplay and that kills the mood to replay the game, I finished it but the replayability value is zero. BioWare is fucking up!

Worst game lately...DAI It's not a bad game, but it's the worst compared to others that I have played recently.
Of all time I can't say. There aren't a lot of games I have played that are awful, I guess I'm just lucky? I just stick with the genre I like and pick recommended games from that.
Oh I thought of one. I hated Final fantasy X. A lot.

I just did not get the appeal the first time--probably made it past the two of DAO's main quests before I stopped. Flash forward to a couple years later, and it just clicked (not just that game but the whole DA universe/lore/flavor).

Lately? The Witcher 3. I bought it 'cause of the hype and without knowing anything about it. Only got to 8 or so hours before I unistalled that shit. Wish I could get my money back. I have this to say to reviewers. I also tried to play Fallout New Vegas, but I didn't like the map and aiming/camera system. Perphaps I'm just bad now at FPS (haven't fully played one in years), but I couldn't find or kill anything. Of all time? FFXIII. No doubt. What a piece of shit in every way. I even got a console borrowed to play it because I had completed all FFs so far, but I'm not touching that franchise EVER again. Luckily, Xenoblade restored my love for RPGs
