I went into DA:I ready for some companion to be a bit sketchy, and I remember raising an eyebrow at Blackwall once or twice as his tales seemed a bit womped... but Solas ended up taking my complete attention for obvious reasons. But, I rarely took Blackwall on missions and spoke to him in the barn only when I remembered or cared to- but never eagerly sought out his presence. In fact, on my first play through I totally missed the part where he went to prison with a false identity, etc. So on my second playthrough when all that sh!t when down, I was completely taken off guard but not surprised.

I realised almost from the get-go that there was something 'off' about Blackwall's past and that he seemed not to have the answers a Grey Warden should.
As with so many people, it was claiming to be in Ferelden during the Blight that initially got my instincts triggered. If he had, he would have been summoned to Ostagar by Duncan. Failing to follow those orders would be akin to treason or something like that.
It wasn't hard to figure out he'd done something bad in the past, something which caused him great shame but hey some Wardens do have shady pasts. Another thing that set off some more doubt in me was how quickly Blackwall felt personally part of the Inquisition. He went on about how good it felt to be part of something important. I think there's even a dialogue option where the Inquisitor can point out "We already" and "Being a Warden wasn't enough?" Or something like that.
Another red flag for me was the discussion on how to kill an Archdemon. You can even hear it in his voice when he says "it has to be a Grey Warden sword." I did wonder if Blackwall was lying to keep the true method of Archdemon slaying a secret, but never once has any Grey Warden or rumour ever claimed it has to be a Grey Warden sword. Its always "Only a Grey Warden can kill the Archdemon."
Another red flag was when we learnt about the false Calling in and around Crestwood. Every Grey Warden in Ferelden and Orlais was affected by it. Even Stroud was affected by it and yet when I talked to Blackwall in Skyhold about it he claimed it wasn't even bothering him. I found that rather odd. (Mind you, if all Grey Wardens can use the taint to sense darkspawn, can't they also sense each other? Makes me wonder why Stroud didn't pick up on the fact Blackwall wasn't tainted if you bring him along to Crestwood.)
So by the time Wicked Eyes, Wicked Hearts came around. (I always do the Western Approach/Adamant fortress part first.) I was certain that Blackwall had done something awful in the past he was ashamed of, and I wasn't entirely sure he was actually a real Grey Warden. I was somewhere between thinking he either killed the real Blackwall and took his identity, or he was something like Anders. He'd done something unforgivable as a Grey Warden and run away from the Order.
As a result I wasn't... well I wasn't surprised when his personal quest came up, to learn he wasn't a Warden. Only a Warden-recruit. But I was surprised he had murdered innocent men, women and even children for gold and ruined the lives of his unit by lying to them about what they were doing. That did surprise me. I didn't think he was THAT evil.
As a result all but one of my Inquisitors (the one that romanced him and thus had him pardoned) decided the real Blackwall had the right idea. Give Ranier back to the Grey Wardens. Personally I'd have preferred giving him to them and having them force him to undergo the Joining as part of his personal quest rather than the whole "serve the Inquisition first until Corypheus is dead but.... ahh well. Good enough.

I wasn't suspicious until that scene where you meet Hawke's warden friend in the cave. I still believed he was a warden but his story seemed more weirder and weirder as the game went on. That's some good writing.

Where’s the “I saw the leaks before the game came out” option?

I used him a lot on my first playthrough (and subsequent ones ), so there were definitely hints that he wasn't being completely truthful about who he was. As others have mentioned, him claiming he was in Fereldan during the blight was my first clue that something was off but it was also the way he spoke about the Grey Wardens. He spoke highly of them but always spoke of other Grey Wardens like they were different than him. Also whenever I asked him anything about the Grey Wardens, his answers were very vague. He would say things like "We do that don't we?" as if he was asking me for confirmation about his own order. However, I did not suspect that he was a convict pretending to be a warden. My earliest guess was that he simply didn't see eye to eye with the rest of the order but still respected what the grey wardens stood for. So I was still blown away when the truth came out.

Not only was he in my party, I was romancing the Dude. But since I already knew all about Wardens, I never took any of the Warden questions. Duuuuh- those questions were for the Newbz! And I wasn't a newb! I'm so amused that face the Inky makes in VR when Rainer confesses was exactly my face. Man, I could have been blown over off my chair.

I wasn't suspicious at all. The guy gave a speech in front of many Wardens and no one called him out so I didn't think much of him not being a Warden.

It was very clear that he was a fake from the start to be honest.

I think I knew something didn't add up from his "I was in Farelden during the Blight" line, but I didn't think it was that much of a deal, so I largely ignored that. Big mistake For me i twas during Varric's quest as I thought Grey Warden if we're heading into the deep roads to this thaig that Varric went to with Hawke it'll be handy to have a Grey Warden to face down the darkspawn and when he said he didn't know how many there were that's when it clicked because Alistair said Grey Warden's can sense them as it's part of who they are they have a connection through the Blight and all that. He tells you all this when you become one in DAO. As even early on when I first did that quest on DA2 I always took Anders with me for the same reason because of his ability to sense them. That for me is when it clicked that something's up with this guy because if he were a true warden he'd have known these things.

I started off with Inquisition first, so I didn't have any meta knowledge to warn me off as to what he was lying about. His brooding and trying to warn the Inquisitor off in a romance was a pretty huge red flag, along with Bull's observation that he didn't really believe what he was saying. The guy was practically a huge neon sign that something was off, but I had no idea as to the level of how bad it was.
In the end, I believed he was sincere about wanting to believe there was redemption, but I had no clue what he was sorry for.

Not at all but I also found him the most boring of the companions, even after his revelation so I never really paid him much mind.

Not at all...first playthrough I never managed to trigger his side quest, and probably missed some dialogues. He just stayed with the Inquisition the whole time and nobody was the wiser. He mysteriously doesn't show up in Trespasser of course, in that scenario, and nobody mentions him.
