During the Anvil of the Void quest, my Surana took Morrigan, Oghren, and Leliana. I turned the list of names in Caridin's fortress over to the Shaper. I talked to Shale at Camp and told her about Caridin and how she was once a dwarf. She wants to explore her past but can't remember where. Is there anything I can do to spark her personal quest?

You have to go to the Cadash Thaig. If it doesn't appear on the map go to any other Thaig with Shale in the party and she should remember the location.

As I recall, she just brings it up in camp. Maybe your approval needs to be higher? My Cousland didn't bring her, so it had to come up that way.
I've done her personal quest both ways. It's more unsatisfying to have her hear it all second hand, to be honest, so I always intend to bring Shale on the Void quest going forward.
Edit Add: Oh yeah. I think I talked to her at the Shaperate and she just brings it up automatically. So, it doesn't have to be camp talk.

After the conversation about Caridin's fate, just bring her with you to any place in the Deep Roads and then she should enter a conversation where she remembers where you need to go.

After talking to Shale in camp, I took her to the Deep Roads and that jogged her memory and uncovered where Cadash Thaig was.

Then it was wrong, that the Warden need to speak with her in the camp before? Hmmm.....my PC did talk to Shale after completing the Paragon Quest and she was extremely interested in all that happened and hoped that it would shed light on her origins, but when talking about her vague memories, she had no idea of where they occurred. I then took Shale to Aeducan Thaig and it was there that she remembered that she came from a place nearby and marked it on a map. So, was the Party Camp discussion necessary? I don't know for sure; on this PT, I didn't take Shale with me for most of the "Paragon of Her Kind" quest. (In past PTs, I took both her and Oghren for most of the quest because I figured it was important to both companions. And Shale would then be told who she was by Caradin. I'm surprised Caradin recognized her given the way she was supposedly "shrunk" by Mage Wilhelm.) Would just taking her to Aeducan Thaig been enough to jog her memory without the more lengthy camp discussions? Don't know.

Then it was wrong, that the Warden need to speak with her in the camp before? So, was the Party Camp discussion necessary? I was pretty sure that is the only place she will talk to you about Caridin's fate. And then say she vaguely remembers somewhere in the deep roads. This is of course a different conversation than the other one where she talks about Wilheim finding her in the deep roads.
