In all my playthroughs, I haven't been able to do it. The maps would be completely explored, yet I would still have dozens (in total; especially in the Tales section) of missing entries. Are there ones blocked off because of certain choices? I'm currently on a run through to do that.

There are a few that aren't unlocked depending on the world state you have and the race of your Inquisitor for your Inquisitor it only unlocks the one for that character (Adaar, Cadash, Lavellan or Trevelyan) also one depending on Ferelden's ruler Alistair or Anora (you unlock both if Alistair is still a Warden), if both rule together you unlock only Alistair's Loghain also only unlocks if he's still alive, but only if he is the Warden contact you might also want to check the wikia page with a completed playthrough and jot down the ones you missed and where or how to unlock them

Have you visited the Val Royeaux book merchant? While you may have scoured the map, it is still possible to miss some things. Especially in temporary quest areas. And that's where the merchant comes in.

It is not possible to get them all in a single game. For example, if you choose mages, you won't get the Envy codex - you only get it from the Templar quest. It isn't available from the VR merchant.
In any game, you will only get codex entries for the specializations your class is eligible for. The other 6 aren't available.
This is one thing I hope they remedy in the next game. Reward meticulous exploration by letting us have access to every codex entry in a single game, assuming we can find them.

Hmm... there is also the Well of Sorrows decision. It has been a while, but if I remeber that right, one can get the codex entry on the list, but the real 'text' only appears if Inq drank themselves. 'Tis a bit dissapponting.

Yep. Still, at least they are findable and the slot is filled out in the codex. Being unable to read them due to a RP choice is not near as bad as them not even being available in the game.

Are you referring to the "untranslatable Eleven text" codex? Yes, there are 4 of them. One in the surrounding area and 3 inside the temple.

Is there a way to see all the codexes you have opened over all the games you've played, or is it only the ones you've opened that playthrough? I haven't been able to find any way to see every codex I've opened so I can't tell what I might be missing in the playthrough.

It's just a shame, it's understandable...but a shame. It would've been nice to see it saved even in the Dragon's Age Keep. Not all the writing, just a record of even the codex numbers you managed to collect. I know I haven't collected all of them, I was just wondering how many I was missing.
