This is based upon a thread in the Mass Effect forums. I thought it would be fun to start it here as well.
Many of us have played and replayed games in the DA series. On those playthroughs, there are usually those little things we didn't catch the first time around. Those little things are what draw a lot of people back to the games, myself included.
I'll start: The cave of bear heads in Emerald Graves (iirc.) I don't have a screenshot, but I wonder if the devs hated bears while testing the game as much as us players eventually did?

Didn't see the massive army of the Inquisition from the ramparts of Sky hold until my third play through.
Apparently, the war table mission where a noble girl and her common boyfriend can be kept together can be seen getting married in Val Royeuox.
Sera will open the cells below Haven for you. The stuff down there doesn't seem so great anyway so I've never cared. Haha
Sutherland and Company line of War table missions ends up having them gather in the Sky hold Tavern and you can see them there.

I completely missed Quizquisition guy my first time through, though I know he's pretty well-known generally. I can't think of anything else right now. I came to the series late, so most of the secrets I accidentally spoiled for myself on my first playthrough. Not all of them though, evidently

I really enjoyed my third play through, so at the end, before Trespasser, I walked around the edges of Sky hold checking out the horizon and looking down and there they were. A thousand little tents in the distance.

There are so many, I'm not sure where to begin. Hmmm, most of these are covered by the easter egg thread, but I'll list them anyway: - Lord Trifles Minutiae
- Cheese shrines
- Farmer Giles, formerly of Ham, now of the Hinterlands
- The Unattainable Holy Grail
- "Don't touch my pyramid!"
- The multiple dialogues between the sergeant and the guard near the trebuchets in Haven
- Death defying leaps from Leliana's rookery onto Solas's head
- Sera's Cabinet of Wonder Whose It Was
- The Bellanaris Din'an Heem arrow sticking in the roof of Skyhold -- still have no idea what that is about
- The door connecting Cole's loft to the upper ramparts -- didn't discover that until like my fifth playthrough!

If you click on the notice board in Val Royeaux, you get some ambient dialogue with Cassandra regarding Templars.
Only discovered this in the past week.
(I think it may have to be done before meeting with Mother Hevara, which is how I've missed it.)

If you do the Winter Palace Quest line first adn get Morrigan, then you can listen to a conversation she has with Alistair/Loghain if they are Hawke's Warden Contact in your world state. I actually knew about the discussion before my most recent play through, but then didn't think about the timing, accidentally completing the battle of Adamant and the Fade "as I always do" before the Winter Palace, then realized I'd missed my chance. Loghain never makes it to DAI, and I love my Hawkes, so I've only played once with a world state where Alistair is a Warden.

The dwarven statues downstairs in Skyhold. It's by the basement library. One man, one woman. It makes me wonder if the dwarves used it and/or helped with it's construction in some way.

While it might be a case of the devs reusing assets, replaying Origins made me notice a lot of Eluvians dotted around the place. There's several in the Harrowing chamber, in the Fade, Soldier's Peak, the Blood Mage hideout in Denerim etc.
That they really might be Eluvians does fit with how Trespasser depicts several as having been bought by wealthy patrons, who viewed them as a historical curio and novelty item.
Many of the locations featuring them in Origins tend to have mages present, who may have recovered them because they realised they were some kind of magical artefact. Eluvians being something often found in ruins would explain how Duncan knew of their existence (although mistook them for Tevinter creations) during the Dalish Origin.
Morrigan may have had such trouble finding one because so many had been plundered by treasure hunters over the centuries, it left very few undisturbed locations where she might be able to locate an Eluvian that had remained in situ.

I'm copying myself, cause I'm lazy So I was reading the TV Tropes page of DAI for fun, when I stumbled over this text: my head went: /Evanura sword? I never heard of an Evanura sword. /So I go to the Dragon Age wikia and what do I find? EvanuraAcquisition:Can only be acquired after finding the codex entry The Silver Knight in the Exalted Plains. Once acquired travel to Emerald Graves and interact with the Lindiranae's Tree landmark located in the same area as the roaming Giants. Upgrades Comes with:
Master Frost Rune icon Master Frost Rune (+16 Cold Damage) Bugs:When claiming the specific landmark for Evanura the player may not receive the sword. This is usually due to the player not obtaining the codex entry: The Silver Knight, either prior or after the claim. Simply grab the codex and claim the landmark or, if already claimed, return to it. Four finished PTs, four PTs where I did everything and I still didn't know this sword even existed.

I have found the codex entry and always mark the landmark, but I don't think I've ever found the sword either. Probably because I'm usually more worried about hiding from the giants in the area.
Edit: Got it! After claiming the landmark, the glowy search ring appeared. I hit search and a loot node popped up. Didn't have to move or anything. Also of note, killing giants at levels 16-17 is much easier than at level 19+.

I really wish we had a schematic for Evanura, so we could give our sword and board Lavellan's something to go with the Sentinel Shield. Shame that the only Elven sword schematic we get is for the 2H Sulevin Blade, couldn't they have included a 1H version as well?
(Would even have fit with quest in which we get the Sulevin Blade, since Dagna reforges it from the fragments. Should have been an option to ask her to craft us either a 1H or 2H version, like we had with Starfang and Vigilance in DAO and DAA)

Apparently Dog in Origins will lick your party clean of blood. Who doesn't want a tongue bath to look spiffy for those Polaroid moments, especially when you meet the Queen.

Finally did a playthrough with Champions of the Just and did't do the flags, which surprisingly led to Abernache not dying. I had no idea that could happen. Also managed to keep Barris alive, which I never managed before.
I'll have to keep an eye out for this Evanura sword because I had no idea it was a thing.

Npcs on val royeah talking about thom rainier. Npscs talking about josephine being exiled before you she tells you that. Codices in exalted plains and emerald graves regarding some underground operations are connected! Side-quests are not so irrelevant You can see the watchtowers the inquisition built in the hinterlands for over watching the refugees, same for emprise du lion. Dorian NEVER gets old. Sοmething i always do but some of you may not know, on storm coast you can craft the amulet so you can take blade of hessarian on your side after you defeat their boss in their stronghold. Afterwards no more fightinh with them on the map. plus you get agent. god this game sure has quite the effort in it by the devs!

TIL, that the Dalish mage we encounter in the Hinterlands that begins the quest line, Measuring the Veil (Solas) is the very same Dalish Mage named Mihris; the lone survivor of the clan Imshael slaughtered in the book, The Masked Empire. Measuring the Veil

I noticed this while playing through Wicked Eyes and Wicked Hearts last night. The part where you get to the royal wing and meet the guard, who's tied to the bed, at first, my Inquisitor left him there. But when she came back into the room, I got another cutscene and the guard swore at her, "You piece of shit! Let me outta here!" I was like, 'Too bad, buddy! It was your fault you fell for that trap!' Another thing is if you talk to Cullen about his lyrium addiction before going to the three warrior specialization trainers, you get an extra dialogue option with Ser (Templar trainer). You can mention Cullen's thoughts about being a Templar and lyrium.

This isn't anything I've found but just noticed. Finished another main DAI pt and for the first time Leliana became Divine. It was actually nice to hear her voice change back to the old Leliana from DAO, her tone sounded light and happy. After the entire game where she seemed stern and stretched almost defeated, it was nice. Amazing VO and direction. And bonus, as Devine she freed the mages. That might have more to do with your "softening" her than making her Divine. I've seen the hardened Leliana Divine referred to as "Murder Pope".

Interesting, didn't know you could harden anyone in DAI. It was a relief to hear her old self back. Have done the hardened Leliana and her voice sounded sad. It hinges on two things. Tell her not to kill the traitor spy and tell her Skyhold volunteers aren't war fodder. Then she doesn't kill the spy nun and you got yourself a soft Leliana that may or may not become Divine depending on other things. I got murder pope my first game and was quite horrified. In a "I made Bhelen King" kinda way.

3rd playthrough since launch and just found put this: On Skyhold on the upper tremch with the view in the valley there is a prompt to push "x" (ps4 here) ,a codex opens mentioning that the inquisition troops cannot all fir inside skyhold and in this regard they are located on the valley beneath. So i looked in the valley below and VOILA: little tents and campfires!

That codex changes a couple of times throughout the game so make sure to check it again later.
