Tomb Raider. There are a few scenes in both of the new games that imply Lara might get raped. Those scenes were unnecessary and sickened me.

Also, you would typically think Kratos is the one that doles out the harshness, however, in Ghost of Sparta he gets the mother of all dick choices: freed from his sins and depowered, finally with his daughter, he has to give her up and take up his curse again or doom all of existence, including Calliope. You get to re-murder innocent souls and forcibly push away the only good thing in Kratos' life in order to stop the apocalypse (of the day). And for bonus pointlessness, Kratos eventually destroys most of the world anyway in GoW III and gets to re-experience that loss with Pandora. And now this new game is pretty heavily forshadowing even more dickish filial loss.
And again, not going to spoil anything but both Darkness games make you kick yourself in the feels, really freakin hard, though it's fully apparently only in subsequent playthroughs when you know what's going to happen. Check'em out.

#1. When I had to walk Clementine through chaining me (Lee) up and shooting me (Lee) after giving her underwhelming survival advice.
The following were also rough on me.
When Clementine had to decide to kill the dog after I'd just played fetch with it.
When Clementine had to decide whether or not Kenny had done something awful to the baby.
When Riordan told my Warden she wouldn't get a happy ending with Alistair.
Pretty much end of Act 2 and all of Act 3 of Dragon Age 2 was trying to destroy my soul.

I didn't see Telltale's GoT series in here yet (maybe I missed it). True to the books/TV-series, the game has various instances of both physical and mental/emotional cruelty, many of which are of course due to all our favorite character: Ramsay Bolton. You just know, when this guy enters a scene, things are going to get really bad for the Foresters. But he is by far not the only one. I hardly ever saw a game that strings so many acts of cruelty after one another.

True to the books/TV-series, the game has various instances of both physical and mental/emotional cruelty, many of which are of course due to all our favorite character: Ramsay Bolton. You just know, when this guy enters a scene, things are going to get really bad for the Foresters. But he is by far not the only one. I hardly ever saw a game that strings so many acts of cruelty after one another. I don't think a game has managed to piss me off more than Telltale's GoT. If there's a second season I have nil interest in it.

Cruel for me as a player, or for the character experiencing it?
For the former, there are a number of examples of having to do something that seems morally wrong or dumb in order to make a game advance.
For the latter-
Psychologically, it might be the ending of SOMA, where Otherwise, maybe Jason Brody's hallucinations in Far Cry 3?
Physically, the most obvious would be Isaac Clarke having to have that needle stuck into his eye in Dead Space 2, Lee having to cut his arm off in The Walking Dead Season 1, and Clementine having to stitch her wound without anesthetic in The Walking Dead Season 2.

One of them for me has to be the end of Crisis Core. For those that don't know, Final Fantasy: Crisis Core was a prequel to FFVII, released on the PSP. In it, you play Zack Faire, SOLIDER First Class. For those that might not remember FFVII, Zack was Cloud's friend in SOLIDER. He was dead and had been dead for years. I'm not a huge FFVII fan and in fact, I hadn't even played it prior to playing CC. I knew enough of the story though. I knew about Aeris/Aerith (I wasn't under a rock,) and I knew about Zack. Even knowing that, the game got me sucked in. Good story, overall excellent voice acting save that tool Genesis. Fun combat and surprisingly beautiful FMVs. There wasn't many, but they were almost PS3 quality. Sephiroth was actually very well done as both a hero and as a villian. This is coming from someone that thinks that Sephiroth was a very overrated and lame villain when I played FFVII right after finishing Crisis Core. Then I saw the ending looming before me. The game makes it pretty clear when it's almost over. Basically, you have to fight never ending waves of troops. Zack's memories begin to crack and break before your eyes. This happens even f you get hit by just one guy. You can fight as long as you want, but eventually you'll lose the ability to heal and/or cast spells. Even if you're just giving up, it takes some time for the DMW (Digital Mind Wave) to finish breaking down. You're helpless as Zack is overwhelmed. The final cutscene is a tear jerker, and this song is playing the entire time. I still sometimes hear Zack in my ear, saying "I'm trying! I'm trying!" from the combat practice sequenzes with Reno.

If unbelievable levels of annoyance count as cruelty, here's one I hadn't thought of - escorting the escaped slaves to the Lincoln Memorial in Fallout 3. MAN was that a pain in the ass - they were constantly veering off in the wrong direction and getting themselves into fights. Eventually I just reloaded, fast-traveled, and waited for them to show up. Apparently the game will just "teleport" them there under certain circumstances.

The Witcher 3 has some disturbing scenes. I think finding Junior Whoreson, hunting the murderer who attacked the bard, finding the last victim, slaying the unborn child... I'm sure I'll encounter more sick crap before I finish the game.

The Last of Us.......... there is a scene where you are trying to smuggle Ellie out at night and cops stop you...... One of those cops was voice by Jennifer Hale and you have to shoot her in the head to escape...that traumatized me. I am so, so sorry FemShep.
