Greetings,
I do not understand why during the duel between the Inquisitor and Otranto why the Inquisitor has to publicly declare his or her love towards Josephine for the romance to continue.
I mean, when the Inquisitor and Josephine were making out before and dancing (in Halamshiral), I am sure they would have said their "I love you" statements so yeah, I don't get why we should publicly declare it. Shouldn't Josephine know that the Inquisitor is in love with her by that point in the story ?

It's just giving you one last option to back out and start the true relationship with Iron Bull before you are locked in with Josephine for forever. --- Or less likely, and far longer thought: A few make out sessions in the public park do not equal love. Even if the person tells you they love you. Because you are as safe as you can be, given the situation. Things are going good, there is no strain on the relationship, so not saying I love you will just interrupt an otherwise nice arrangement of regular exchange of saliva.
And Josephine might be "innocent in love" but I'm pretty sure she knows plenty of stories of high ranking people messing around with someone of lower rank then tossing them in the dirt afterwards. Maybe in the back of her mind she has the same idea as Otranto "The Inquisitor liaison with her is an affair of passion or convenience." She is after all, on the way to the war table and a very convenient transportation to the gardens.
Anyways then Quizzy (who is the last hope against the giant, demon spewing hell hole) goes off to risk their life against a rival she were in the process of putting down diplomatically and expressly told that Quizzy "NOT TO START A FIGHT WITH OTRANTO". So she's thinking, what the hell is wrong with them, starting a fight with a person I was already dealing with. And they even handicapped themselves, using a weapon they are not familiar with, against a master duelist. So she's furious, the last hope for Thedas, throwing their life away on a quest that was already being handled in a harmless manner. She's probably expecting them to give the other answer: "Apparently I didn't think this through". Then she'll give Quizzy a good scolding, stop her relationship with a stab happy individual who will pick a fight with anything and continue on breaking off the engagement with Otranto. But then boom, Quizzy suprises her, they were doing this because they loved her and that syrupy sweet talk they had in his/her quarters might have been genuine. The fairy tale romance might be real, so better lock in the deal with that snuggly little nug before they run away and get skewered, tossed into soup and served at Delilah's Howes dining table.
At least, that's how I see it.

To be honest, I didn't think this duel was necessary if Josie was already in a relationship with the Inquisitor. She could've told her parents about him and they wouldn't have gone through all the trouble to plan an arranged marriage with another guy. I mean.. her sister, Yvette, met the Inquisitor during WEWH, so it would've made sense for the rest of the family to know. The other thing is if the Inquisitor loved Josie, he would've confessed his love to her directly instead of running off to fight a so called noble, who's below his rank and power.

The other thing is if the Inquisitor loved Josie, he would've confessed his love to her directly instead of running off to fight a so called noble, who's below his rank and power. My thoughts exactly. There is nothing stopping the Inquisitor from directly approaching Josephine's family to ask for her hand. The Inquisitor is far more powerful and influential than Lord Otranto.

Some other Inquisitor might not have made such a statement to her yet. If it's not in the game, then it is up to individual player headcanon. Also, there is a difference between a private, personal moment and a public declaration. A public declaration can be seen as more official, telling everyone that this is the one I love and want to be with. That IS meaningful to some people, both women and men.

My question is, if the reason Josie gives for her inability to hook up with Blackwall is "class differences," how is Josie able to hook up with anyone other than a human noble PC? I'm guessing because the Inquisitor is the head figure of the Inquisition which becomes a rather powerful organization.

The excuse for that is the same as a lot of the race and class differences in both DAI as well as DAO: the PC's role as Inquisitor/Warden transcends all of those other considerations. On the other hand, Blackwall is a commoner who has the addition of the great dishonor that is later revealed.
