I just defeated the dragon in the Western Approach. I found a small cave in the area that goes down underneath the large blocks. Nothing special under there (as far as I can tell) but an interesting find.

There should be a mosaic piece.

First time I played through DAI I completely ignored Solas's necklace. Now I can't stop seeing it and thinking how the heck did I miss it and all the other hints.

There should be a mosaic piece. Yeah, that was there. No major loot. Funny enough, I have never completed a single mosaic. Not invested enough to track down all the pieces. I no longer look for shards. I used to collect most of them but there are some that are borderline impossible to get. Not worth it to me.

I'm actually considering doing a run where I don't recruit anyone. That is, not go out and find Blackwall, Vivienne or Iron Bull. Dorian and Cole come to you so they would be fine. I wonder how much would be ignored. I also wonder how things would play out without recruiting Iron Bull. Vivienne definitely can't be Divine if she's not recruited. No Grey Warden treaties. Anyone done this?

It wasn't until my third or fourth playthrough before I realized the significance of the statues in the Temple of Mythal, particularly the ones that are both a woman and a dragon.

I wonder how much would be ignored. I also wonder how things would play out without recruiting Iron Bull. Vivienne definitely can't be Divine if she's not recruited. No Grey Warden treaties. Anyone done this? You can still outright refuse Dorian and Cole. As for Bull, I don't recruit him and it makes little difference. Most of what the followers add are war table operations; there will be significantly less if you don't recruit a bunch of people. On my single Trespasser play, I had recruited Bull and saved the Chargers, but I don't play with him regularly. As far as I recall, he had one pertinent line about the Qunari activity and goings on when I went to talk to him, but it wasn't anything significant. I don't imagine that not recruiting him at all will change things in that regard.
Also, I believe that Krem stands there in Skyhold if you refuse to talk to him in Haven. So be aware of that.
But to answer your question, no. The only person I don't recruit is Iron Bull, and I have a RP reason for that. I also wait until my guy is Inquisitor so the decision is his to make; I don't feel that he has the authority to refuse as Herald.

I actually tried finishing the damn mosaics one playthrough, and still didn't manage to do it. I seem to miss pieces no matter what. Last time I played, I tried not to collect any shards at all, so that they wouldn't clutter my map. I went nuts for them my first playthrough, though.

I would have been happy just to finish one so I could hear what the dwarf in Skyhold had to say about it.

I jumped, hopped and skipped uphill through a steep hill crevice in the Storm Coast to find a Dwarven cave (that did not show on the map) and some moderate loot. As I recall, it was centered just inland from the northern coast.

But no matter what happens, recruited or not, she become "Grand Enchanter" in the new Circle, or makes her own Circle... (If I know well)

But no matter that the Inquisitor recruited her or not, she become Grand Enchanter (if not Divine).

Anyone done this? I've done that. I even told Dorian to get lost. When it came to Trespasser, Dorian shows up as ambassador. I used Varric and Cassandra for the dlc. At the end, Dorian is seen with the others after encountering Solas.
Not much is missed if the others are not recruited. I know it saved a lot of time by not having to run from one spot to another talking to everyone at Haven and Skyhold.

Leliana has a line of dialogue and a question when you don't talk to her at Haven but do talk to her at Skyhold. What does she say? Paraphrasing "So you finally came to see me. I suppose you've been busy. But after the events at Haven, I suppose we all look for new friends." And then the Inky has three options. One of which is like, "new friends are good". And then it picks up from there as per usual about her regrets not leaving her spies out in the field longer.

If only that Leliana conversation had gone something along the lines of; Leliana: So you finally came to see me, I was beginning to think you were avoiding me. Inquisitor: To be honest, I find you a little intimidating. Leliana: And you've been face-to-face with Corpyheus and his Archdemon, so I can only take that as a compliment. Because let's be real, Leliana is a little scary at times in Inquisition. (Wait... she's not behind me, is she?)

I went an entire game not talking to Leliana (other than when forced by cutscene) and she ended up as a very ineffective Divine.

I went an entire game not talking to Leliana (other than when forced by cutscene) and she ended up as a very ineffective Divine. Ineffective how? The thing with Leliana is that, by doing her quest, she becomes convinced of the rightness of her actions, regardless whether she's the fluffy version or the murder pope version. If you don't do it, she does not have the same conviction with which to persuade and/or intimidate people into going along. Without that conviction, nothing she does takes hold and any opposition is a lot stronger/more effective.
