Kotor 1 Dark side Star Forge
There was a special feeling storming the Star Forge with Bastila, both with Sith robes and red lightsabers. Cutting through countless enemies, watching Malak squirm as he realized the walls were slowly closing in on him. I actually felt like a Sith Lord, one of the few games where you are not just a villain, but you are THE villain of the story.
Other than that: ME1: Virmire - Loved the tropical beach setting, really stood out from the other environments, and had great conversations with Sovereign and Saren, understanding them better, and the Sovereign reveal has to be one of my favorite moments in a Bioware game. The Ashley-Kaidan choice was a bit weak, I didn't care about either, so I just went back to the bomb to ensure it went off, which meant I saved Ashley.
ME2: Suicide Mission - No explanation necessary.
DAO: Battle of Denerim - All your hard work came together in what actually felt like a battle.

Off the top of my head - A lot of the Citadel DLC areas for me3. (Not the places you go muck around in, the areas where you kill the bad guys) I also liked the Leusinia Ark in MEA the final boss fight area when the windows are blown out.

I fucking hated those traps at Durlag's tower. I can agree with that they were punitive and you had to toe creep around them, would be awesome with a more modern game engine i think.

If only the 'hold' command actually worked like it should, this dungeon wouldn't be such a chore. I dread it every time just for that reason.
For side dungeons I prefer Dirthamen's Temple or the Still Ruins.

Suicide Mission had awesome music. Perhaps the best of anything Bioware has done. But the mission itself was rather simplistic given the buildup.

I'm a huge fan of the Deep Roads and the Fade.
My favourite locations in the Deep Roads would be the ruins of Cadash Thaig in Origins, the creepy Primeval Thaig in DA2 and Heidrun Thaig, the Underground Ocean (Bastion of the Pure) and the Wellspring from the Descent.
I think the Library in Trespasser has to be a great representation of how weird and strange the Fade levels can and should be, where even something as simple as "up" is relative depending on where you're standing. I'd like to see future Fade levels play with that aspect more, similar to the Liam loyalty mission in Andromeda where gravity shifted so that ceilings became floors etc.
(Also Future Redcliffe in Inquisition is always extremely creepy and atmospheric to play through)

Visually, I would have to say the last few levels in Descent. The ocean was a very nice discovery upon first encountering it, and the Wellspring took my breath away (I just wish we could have explored more of it, as it looked gigantic!).
As for general level/quest design, my favorite is still the very last run and fight leading to meeting with Solas in Tresspasser. By that point, the anchor is completely unstable, forcing us to constantly keep an eye on the focus meter. The fight music made that last fight against the Qunari mage feel that much more desperate. For me, it was a completely immersion in the game and in my Inquisitor's head at that point.

My favourites include:
The palace in Jade Empire, both when you first enter at the end of chapter 3 and the very end of the game. The build up to everything in both sections is superb and the music.... oh my the music.... Jade Empire just has such an amazing soundtrack imo
The final battle in Denerim in Dragon Age: Origins. Alistair/Anora’s speech, the army cheering you on, the battle with Urthemiel atop Fort Drakon, it’s brilliant.
When the Qunari finally decide to try and take over Kirkwall in Dragon Age II, that level plus the duel with the Arishok was quite the thrill, more so if you duel him when romancing Isabela.
The battle for Haven in Dragon Age: Inquisition, it was pretty damn intense trying to fend off the attackers and make sure everyone survived, plus meeting Corypheus for the first time was just awesome, the music for that whole level was some of the best I’ve heard in a video game.
Pretty much the entirety of the Trespasser dlc for Dragon Age: Inquisition, that hefty information dump from Solas at the end answered so many questions, Solas’ theme song is amazingly heart wrenching and the vow to either stop him or redeem him was a nice touch.
I need to replay a lot of BioWare games now...

DA:I Old Library was nicely done in Trespasser, but my absolute favourite is Liam's mission in Andromeda, easily because of how the ship shifts and its greatly done for the pieces moving around. Also liked ME1 Citadel.

If I need to pick just one... I think I'll go with Virmire. That was a very special part of one of my absolute favourite games of all time.
Debating with Wrex. "Hold the line!" . Approaching the base, taking out all the defences to help the main force. Entering the base, discovering secrets. Sovereign. Making the choice. Facing Saren.
What a great mission. A+ top tier gaming.
