Currently, I am not playing any.
Games I have liked include Neverwinter Nights I and II, Dragon Age 1 & 2, Everquest (but I will never play it again), City of Heroes, SWTOR, Knights of the Old Republic, The Civilization series, Railroad Tycoon I and II, The Sims, Rome: Total War 1 & 2, Medieval: Total War 2(it had weddings!) and Victoria.
What I want is a rpg with a customizable protagonist or a not very violent sim game. I really do not like rape, am not fond of unnecessary violence and do like romances.
I have very poor twitch reactions, so games requiring fast reflexes or coordination are out.
I have mixed feelings about Witcher (very pretty but the protagonist is set and thee are cringe bits) and Anthem. I liked early Mas Effect, but less so as the series continued.
Anyone have any suggestions?

You might want to have a look at Pathfinder: Kingmaker. It's an isometric RPG like the Neverwinter Nights games. Pros: - fully customizable protagonist with many races and classes - a gang of companions to chat with, argue with and romance - real time with pause combat - lets you manage your own barony/kingdom - some quality writing by Chris Avellone Cons: - some boss fights have an extreme difficulty spike (but you can adjust overall difficulty at any time) - kingdom management can be a bit stressful as you need to check your capital regularly for problems to fix (which is done by picking an advisor for the task, waiting for a number of in game days to pass, and hoping said advisor won't muck it up) To be honest, I haven't finished it yet because I'm facing one of those boss battles again, but that's just me and my refusal to lower the difficulty

There is such a game (from my favorites again): Evil Islands: Curse of the Lost Soul (1 and 2).
They both have similar parts from the games you have mentioned. You have to fight, you have a team, think about the build. There is a story as you start as a person (a human - there is no choice) who remembers nothing at all. There are different places to go, different surroundings. In general - the game itself is curious as you really do not know who you are, where you are from and it is like a puzzle.
The second part is easier actually but also has its own atmosphere. I like the first part much more (was going to install it again). ---
Know it is not about the thread though really grateful for you as just could not remember the name of one game I (again) enjoyed playing - Railroad Tycoon. Finally can install it.

Fallout 1 and 2. Oldschool isometric rpg with turn based combat set in alternative atompunk postapocallyptic America. Lots of ways to accomplish tasks and your choises determine how people treat you. Very old graphics though. Fallout 3 and 4 are quite different.

The Pillars sequel has romance and the story isn't as bleak as the first one. You can also play turn-based, so no twitch skills required. Other options based on OP list. Sims: Cursader King II (Paradox is making a 3rd one too). Cities Skylines Surviving Mars "RPGs": You could try Guild Wars 2, base game is F2P, but it can be a bit twitchy. Have you tried Greedfall? It has romances and feel a bit like a BioWare game on a smaller budget (budget was $5m Euro, that's less than what BG2 cost to make).

I second the CK2 recommendation—surprised to see someone recommend it, actually—, though at this point I think it's better for someone new to the series to just skip straight to CK3 when it releases this September. CK2 is my favorite game, with hundreds of hours on Steam (it's rare even for games I like to go into the triple digits), but it's ugly as sin and couldn't care less about accessibility. Thankfully, the sequel seems to be fixing those issues. If a procedural medieval soap opera with romance, intrigue, and dynastic conflicts sounds like your thing, there's nothing like CK. The developers have described the franchise as "Game of Thrones meets The Sims," and there's even some RPG elements.
For more traditional RPGs, the ones folks mentioned earlier didn't do it for me, so just in case you also suffer from some of my many afflictions I want to vouch for Divinity: Original Sin 2 for turn-based, open-ended questing goodness (the first one's good too if you can ignore the bad writing), Shadowrun: Dragonfall for a smaller but densely packed experience, and Sunless Sea if a very well written, narrative-driven game appeals to you. D:OS2 and Sunless Sea have romances, though they may be smaller than one might be used to from Bioware.
If you like CIV, I would also forward a recommendation for Endless Legend and Northgard. EL is a fantasy 4X that eschews the typical D&D lite setting of fantasy games, while Northgard is a hybrid between 4X and RTS with a Viking theme—faster than a 4X, but slower than the typical RTS.

Spiders' games, GreedFall, in particular. But all of them might be considered challenging. Obsidian Entertainment games, Pillars of Eternity, Tyranny, Dungeon Siege III, The Outer Worlds. Most do not feature romance, though (PoEII does). Dragon's Dogma. It is an action-RPG, but your NPC companions can handle almost anything by themselves (if you go online and hire custom over-leveled ones). The Shadowrun series. I don't remember, if there was romance.
Edit. There are also several VNs with turn-based combat, Loren: The Amazon Princess and The Cursed Lands (I haven't played it yet).

Yeah, GreedFall has character customization and romances. Roleplay opportunity is limited though. Similar to BioWare games of old, but of the more discount variety. Lots of ambition, not a whole lot of polish. Gameplay is much easier than previous games by the Spiders.

Since you play the Total War Games, perhaps you would like Command and Conquer plus Red Alert. These two are presently remastered (for 4k) by the original Westwood devs and slated for a 6 June release this year. The package includes expansions and music. I played the series up to Red Alert 3 which was quaint and funny; paratrooper Soviet bears anyone? George Takei was in it, taking the role of the Japanese Emperor. You can play either factions in each game. I always make faces at Stalin in Red Alert whenever he threw out orders; yeah yeah, what is it now? There's fmvs in each game. Well acted imo though Takei was taking it over the top for RA3. I think he had a lot of fun with it. If you like stories and choices, adventure games have those. No need for reflexes, only decisions. The Longest Journey has a great story. The Blackwell series, Unavowed, Blade Runner (an old fav), to name some. Blade Runner was released recently on GOG. It has 14 plausible endings, depending on player decisions and actions. It's an old game btw, like Longest Journey but it was worked over by GOG and released for modern gaming platforms.

If you like stories and choices, adventure games have those. No need for reflexes, only decisions. The Longest Journey has a great story. The Blackwell series, Unavowed, Blade Runner (an old fav), to name some. Unavowed is a definitely an adventure game I'd recommend to people who like RPGs, since you can customise your protagonist to some extent and make story choices. I've described it more than once as what would happen if Gabriel Knight had a baby with a BioWare game. No romances, but there are cool companions to hang out with!

Total War Warhammer II is a good one, despite the magic - sank hundreds of hours in.
Divinity Original Sin 2 was quite good, but too long for me.
Civ 6 is actually quite good. Never too late to return to Civ.
Oxygen Not Included is a cute builder game with unique simulation. Groom pets (ahem, food), tech up, launch rockets (never got that far, but the path is the journey)
The Long Dark had Meer and Hale voicing the survivor. Apocalyptic Canadian snow hell. No zombies, but Mr Wuffles (wolf) and the Bear. The cold is the worst enemy. Got a story mode (bit grindy for my liking) and survival mode (here it shines - Hale did good job voicing survivor)

I've tried to get my friends into the game, and I've seen them experience what you're describing in real time. I look forward to CK3 in large part because it'll have a proper tutorial, an in-game wiki, contextual clues, and a streamlined UI without a gazillion buttons staring back at you. Maybe I'll actually succeed in getting people to play it this time around. CK2 is quite flexible, and game rules allow you to curate the experience to your liking. Lots of people seem to love the wacky stuff, like what you mention, but I never cared for it and turn it off when possible. I prefer my playthroughs much more grounded; I'm actually not sure what the "incest bloodline" is. I'd rather reminisce about the time my character lost his kingdom to his exiled uncle and died in prison, only for his young son to be freed from confinement and elected king when everyone realized said uncle was an incompetent asshole, and such.

The Vampire Coast is close runer-up. Hilarious zombie pirate corpses. But make no mistake - they pack a hell of a punch with ranged gunpowder. The even take to the skies with guns. Never has been fielding rotten bodies more fun than in TW WH2.

I tried to play CK2. I sat there for half an hour trying to make sense of it and then gave up. The screen seemed over run by many buttons that I was overwhelmed; erm, what am I suppose to do first? I also came across a gameplay video by someone who was trying out the incest bloodline like the Egyptians. I watched the whole royal family go crazy. Yeah, I know. I've tried to get my friends into the game, and I've seen them experience what you're describing in real time. I look forward to CK3 in large part because it'll have a proper tutorial, an in-game wiki, contextual clues, and a streamlined UI without a gazillion buttons staring back at you. Maybe I'll actually succeed in getting people to play it this time around. CK2 is quite flexible, and game rules allow you to curate the experience to your liking. Lots of people seem to love the wacky stuff, like what you mention, but I never cared for it and turn it off when possible. I prefer my playthroughs much more grounded; I'm actually not sure what the "incest bloodline" is. I'd rather reminisce about the time my character lost his kingdom to his exiled uncle and died in prison, only for his young son to be freed from confinement and elected king when everyone realized said uncle was an incompetent asshole, and such. It started as father marrying properly; a wife from another family. Then the wife died so daddy married teenage daughter. By the by, after some kids, the daughter died and daddy was still hanging on so he married the granddaughter. It just went on and on with the son, etc. I was thinking; this is so FU. The descendants began to turn up with hereditary illness, mental instability, etc etc.

Currently back to playing Civ VI, the options for non-militaristic/non-violent paths to victory are alot more fully realized than in some past games, and are arguably the funner victory conditions as opposed to military conquest/domination. And Civ games are the gold standard for a reason, definitely worth a look if you're inclined towards that genre at all.

Culture and Diplomacy Victories are my favourites, although I also like the Religious Victory. The “Ages” mechanic is fun, but can be stressful as the countdown begins Least favourite thing...? Rock Bands. Who the hell thought it was a good idea to have units you can’t remove (unless you’re at war) that suddenly assault your ears with “music”?

And though I'm usually a Domination guy at heart, I'm actually just starting a new game right this moment to go for my first Religious Victory in Civ 6- I haven't messed with religion in Civ 6 at all so far, besides just securing a favorable pantheon. So I haven't had a chance to try the new "theological combat" mechanics or any of the other new stuff in Civ 6. Going to play as Arabia (who I also haven't tried yet in 6), since I don't want to not even get a religion in the game where I'm very specifically going for religious victory to try out the new religious features/mechanics.

Nice. Get the Civic that lets you build Temples as soon as possible, so you can train Apostles to buff your religion. They’re also far and away the best religious unit at everything else, so once I get them, I usually don’t build anything else. Unfortunately, the latest update in anticipation of the new DLC seems to be causing issues. My game literally just crashed again. Second time in two days.
