If you don't talk to them at all while they're in the hospital, they will not return to the Normandy.

Ashley sure gets upset though if she does offer to come back and you turn her down. I don't think I've ever turned Kaidan down (when I play with him as VS, I'm doing so because I ultimately want his biotics on the team in ME3).
I have had circumstances where it was impossible to shoot her/him and circumstances where it was impossible not to shoot her/him. Lowering the weapons (i.e. using the interrupt) can tip the scale, but it's not the only thing that can. I've also had a situation when I have kept my weapons drawn (i.e. not used the interrupt) and still had them back down. It's a complicated reputation check that also takes in whether or not the VS was romanced, whether or not they were cheated upon... and how it all ultimately adds up is what matters.

I guess I could do another romance if the start of it felt just right and it wasn't like...the very next time I played. In my fictional character grief, I kinda feel like saving the galaxy, all my comrades, but letting Shepherd die. I don't know if that can even be accomplished.
And regarding your question about Andromeda... Definitely not waiting anxiously. Maybe I'd play it a year from now.

A dying man, a dead woman, both fulfilling their purpose, and reaching for the other shore, in another reality all together. It's perfect.
Bao-dur's relationship was similar, but there was never an option to acknowledge love, and Exile sacrifices herself for a wrong man in the subsequent fiction. Not to mention the appalling way the game glosses over the end of Bao's story.
Anders never lets Hawke to be doomed with him and never lets him close enough to have the unity of purpose...
Thane is a swan song of Bio's romances, and the last echo of Jade Empire. From the moment Thane died, ME3 became a wuxia story for me against it apocalyptic background

Good luck with that. Regardless of how it turns out, I think headcanon will serve you in regard to Shepard's fate. I get it, though. If I played in such a way that Kaidan ended up dead I think I'd care less about the fate of my own Shepard.

Oh, just wait for it. I hate the way Kai Leng was written and implemented in the game (I liked him as a villain in Retribution, which was before the salarian curry, he just got butchered as character in the game). But despite the fact that I don't think BW did a great job on Leng, I'll give them one thing: I really wanted to kill this bastard at the end. I am usually not getting very emotional about fictional characters and even less so if it's anger. But with Kai Leng, they did manage to get me enraged to the point where I just wanted to see him dead. So I guess, despite all the issues and cheap shots, they managed that and kudos for doing it BioWare.

That's super lame that they dropped the ball so hard on the Thane romance when, from what I can tell, it's the classiest straight Femshep option from start to finish.
I didn't say I WANTED to shoot Kaidan. I'm just saying that if he'd not have backed down, I would have been willing to. I mean, at that point, it would have been him shooting me so it would have been self defense (or, apparently, a squade mate would have done it which would have pissed me off majorly.)

I have that DLC, but isn't it just going to be a party where everyone else is having a blast and I'm grieving my dead lover all by myself? The game is telling me to go take shore leave, but I'm NOT ready for a party. What is the latest I can do this DLC so my Shepherd and I have time to grieve?

I didn't give Kaidan the whiskey, so I think his romance option is closed regardless based on the romance wiki. He's the ONLY one, outside of Garrus, who HASN'T flirted with me right after Thane died.

One time I sided Templars and made Anders help me kill the mages. it was my 2nd PT and I was still pretty mad at Anders haha In the end, I felt like we both left Kirkwall damned and seeking redemption for our murder sins.

You can't start a new romance with Garrus in 3. Kaidan will message you about eating lunch together, and that's when you'll know if he's interested. Your other options are
