ME1 was the first shooter I ever played and I went in with a vanguard build haha my ME experience almost ended right there and then. Even after actually learning shooter mechanics I still think ME1 is the most challenging, however, they're all pretty doable on normal, even for someone with questionable tracking aim like me. Aim assist is your friend. Although, ME1 always feels like a breeze once you get passed the Luna base around level 20-something.

ME1 on insanity is the hardest but not in a good way.
Game did give me a healthy fear for charging Krogans though.
I think ME2 insanity was the only one that made me rage quit. I was stuck on the last fight with the Praetorian on the Collector ship, and playing as a sentinel (playing it wrong no less) If I had found my groove and was on my go-to class I'd be fine I think.
Didn't bother with ME3 on insanity. If it was anything like Gold or Platinum matches then I was fine going without it.

The older the games, the harder they are. In other words, ME1. These days games, especially console games, are geared toward being as user friendly as possible. The toughest games I've ever played are now considered old. I thought the Inquisition dragons were tough, until I discovered Thousand Cuts. That ruined all the fun. Seems to be like that with every console game, though. The characters become godly superheroes. Dark souls doesn't count. It's only hard because they strip you of everything you gained when you lose. This game would be considered hard if they did that. Any game would. Imagine dying to an Architect and then getting stripped of your skill points and whatever else. Last time I lost a fight to a truly tough boss in a video game, and kept losing, and never ever won, was in Lotro. And that wasn't even a solo fight. That game had some really tough fights. Your team had to know what they were doing, or fuggedaboutit. I think this is generally true, but not exactly for the reasons you mention. Modern games aren't so interested in being "easy" -- although most of them are to anyone who's been gaming for a while -- they're more interested in creating a cinematic hero experience for the player. To the point that ME3 Vanguards, and ME:A Vanguards are basically superheroes. And superheroes don't get killed, right?

ME1 until you level up. Once I got Invincibility with my Soldier and I could take rockets to the face and laugh, but early on it's difficult.
I still remember playing a Sentinel and getting destroyed on the Therum by the geth Armature and his jumping buddies.

I think the most difficult was ME1. Especially the fight with the Asari commandos on Noveria on Insanity! Those stupid barely-visible biotic attacks that would ragdoll you for about 15-20 seconds while the commandos riddle you with bullets... that is so frustrating and I died a lot there. The fight with the Krogan where you pick up Liara is pretty tough, but those commandos, man. Just the absolute worst. I'm so glad they made biotic attacks more visible in ME2-3 and that they didn't have the ability to ragdoll you.

ME1 hands down. And not just because it was the first game of the series with unfamiliar mechanics, although that's a large part if it, too. I have yet to play through Andromeda on Insanity, but among the first three games ME1 was the hardest on insanity by far.

Probably Mass Effect 2 overall. It brought a better combat system and with that came a greater challenge. Mass Effect 1 had overpowered heat sink mods that granted an almost endless stream of snowblind or inferno rounds raining down on your enemies. Which made insanity manageable with good gear.
Mass Effect 3 had some tough spots but I only really recall one or two stand-outs compared to ME2. I guess the second game sort of prepared me for what was in store. I've no desire to play insanity on Andromeda, honestly I only play high difficulty on games I really love. And with Mass Effect 3 it felt like I needed to beat the whole Shepard trilogy on insanity, even with that ending.

None really. My first game is always on "normal" since that is how most developers design their games. My second is on Insanity and with each Mass Effect game it seems the meta for Insanity always changes, but once you understand that meta or break through the barrier the game becomes a cake walk.

Definitely ME1 for me, as well. The Thorian as someone mentioned upthread... Christ, I'd forgotten until just now how much I hated that whole damned planet. I remember just churning through Medi-Gel like a madman because I had Liara and Kaidan with me and they were both about as resilient as a damp tissue. And then falling back on prayers and bargaining when the Medi-gel inevitably ran out.
In contrast, I just did an ME3 Insanity run this week because Origin never recorded any of my playtime/achievements for it, and honestly it wasn't difficult so much as just annoying. Stupid little Cerb trolls and their smoke-bomb spammy bullshit, in particular.

ME3, for some reason I always struggle on Palaven's moon. There is something tough about at least the 1st or 2nd battle on that moon, before you get Garrus. I've never been able to really put my finger on it. But you have to be on you toes.

Well, since playing on insanity during the first playthrough was not possible on consoles for either ME1 and ME2, I find it impossible to make a fair comparison. I do find that all three games of the old Trilogy get pretty easy after only a couple of playthroughs because enemies pretty much always spawn in the same places in the same patterns... making it very easy to just know ahead of time where to take cover and even where to aim next.

I remember Grissom Academy mission on insanity kicked my ass half a dozen times before I got the hang of it. ...mostly because Rodriguez didn't keep her shields up... LOL That dang Rodriguez.
