Most I died in any ME game was always when driving that moon buggy thing.

For me it must have been ME1... no clue about the combat and no proper cover system. From ME2 onward I never had much problem. Even MEA is relatively easy if you play it like a cover shooter. Combat arenas are still set up everywhere and the AI still doesn't outflank or rush you enough.

Probably Mass Effect 1. If you start a new game on level 1 on insanity as an Engineer or Adept you're pretty screwed for the first few hours. I did a playthrough of ME2 as an Adept once and started with an imported Shep, he was level 50 so i started at level 3. That was pretty brutal, the first few missions to recruit companions took some time and some hair pulling. If you do a New game plus in any of the games at max level I think they're all "easy" on Insanity.

ME 3 for me. The Grissom Academy atrium gave me fits especially since Cerberus flanked the crap out of me. Also the missile battery in London was no picnic being swarmed with brutes and banshees while the reaper destroyer kept creeping closer and closer. I found ME 2 was the easiest which is odd because it was structured very similar to ME 3. I think the difference was that ME 3 was much more aggressive. In ME2 all that really had to be done was get in cover. Take out enemies and advance.

I don't think any of them. They were all pretty easy. Then again I am not one of those people who enjoy playing on Insanity difficulty. Normal is good enough for me. It's generally safe to assume that "difficulty" questions are based upon Insanity as the benchmark. I think I struggled, initially, with MEA the most. It wasn't even the gameplay that frustrated me. I expect to die a few times while learning an entirely new game with a "Level Chump" player character. In combination with the save system, though, I got annoyed as hell and dropped to Nightmare, briefly (from around Levels 8-14). I regretted it, since I now have to wait an extra playthrough for my trophy; and I won't feel I've earned it until I achieve it with a fresh, Level 1 Pathfinder. ME2 felt pretty challenging on my first Insanity playthrough, but I didn't get frustrated. ME2 and MEA are fun enough to keep things interesting. Neither of them is all that difficult, now, having learned to play. ME1 was "grindy", since enemies soaked so much damage. I never played ME3 w/o an imported character. It's a cakewalk starting st Level 30, though. Insanity feels much like normal in ME3. The MEA save feature is a fun-drainer, for sure, when trying to learn the game. If they fix the save feature, I'll be happier all around.
