Before the extended cut was released, Anderson, Joker and Ashley/Kaidan/Liara showed up in the flashbacks as Shepard chose an ending. If the player didn't romance Ashley/Kaidan/Liara, Liara would always show up in the third flashback.
After the extended cut was released, Anderson will always be in the first flashback. The second and third one is determined on what was done during the playthrough.
The second flashback would be a character who died during the playthrough. The times I had Liara killed on the beam run, she would be in the second flashback. So I will assume she will always be in the flashback, if dead, unless she's a LI which will be seen in the third flashback
The third flashback is the LI. If no romance, it will be the squadmate used/talked the most
In my second flashback, I've had Miranda, Mordin, edi, Wrex, Thane, Ashley, Legion,James and Liara. edi has shown up the most since I always choose destroy
Note: Depending on the player's playthrough, only Ashley/Kaidan will die in the playthrough and that's on Virmire. If control or synthesis is chosen, I have no idea who would show up in the second flashback
In my third flashback, I've seen Samantha, Ashley and Miranda. The few times I didn't romance anyone, I've had James and Ashley in the third flashback

On my last run-through (no romance in ME1, chose Kaidan, Miranda romance in ME2 and 3, everyone survives the ME2 suicide mission, cure genophage, Geth-Quarian peace):
Destroy - Anderson, EDI and Miranda in that order
I also went back on that playthrough, chose all the same conversation options with the Catalyst and tried Synthesis, and the result was Anderson, Thane and Miranda. Surprised about Thane; even though my Shepard was an Adept, never took Thane on that many missions, though I did explore all conversations with him in ME2 and ME3.

My favorite flashbacks were Anderson, Thane and Garrus but if I choose destroy, I tend to get Anderson, EDI and Garrus. Kinda wish we could game it more like with the Collector Base platform pull up.

Any idea what determines the second set of slides? Somehow i always seem to get Jacob, Samara and Kasumi. As posted in the original post. Its someone who died in ME3. I'm surprised you got Jacob since the player doesn't know he's dead until his name is seen on the memorial wall after completing Cronos. That only happens if that side mission involving him is not completed. Samara and Kasumi I can see being in the second flashback. When you picked synthesis, did any squadmate die during your playthrough? If not, I'm surprised, of the three you mentioned, since Shepard is with the squadmates in ME3 more than with the others. |

Went for Destroy in the end..Jacob was alive and well when i left him after the party, as were Kasumi and Samara. Only the obligatory people died (Thane, Legion, Ashley, Mordin...sniff), even Miranda made it out alive this time. By the second set i meant those largely static colour slides, Jacob's talking to a bunch of people (no idea what that's about), Samara's relaxing in a garden with Falere, and Kasumi's pondering over Keji's graybox. I know Kasumi's changes depending on what you let her do with the box and the ending you chose. They're interspersed by shots of krogan doing stuff and quarians doing not a lot.
I guess it's just "pick three Mass Effect 2 companions who aren't dead ". I'm doing to have to kill Jacob off to see if I can get Jack or Miranda I guess.
