I doubt they would make the game that hard, BioWare focuses more on the story of the game then the mechanics behind it. Combined with the majority of the people that play their games and upload their statistics don't seem very interested in extreme difficulties since a very small percentage finished Mass Effect 3 on Insanity I just don't see it happening.

Combined with the majority of the people that play their games and upload their statistics don't seem very interested in extreme difficulties since a very small percentage finished Mass Effect 3 on Insanity I just don't see it happening. people not interested in difficulties can play normal or casual. But Insanity should be challenging , otherwise don't call it "insane" . i'd like ME2 modes . something is for sure, everyone would love the game to be released asap, but the day it'll come out everyone will wish it had been delayed to make it "better" .

I'd like it to be at least as hard as ME2 insanity was, if not harder.
It's the hardest difficulty. It's supposed to provide a challenge for even most of the good players, and it's completely 100% optional if you don't want that difficulty.

I don't mind there being a level even harder than the current insanity... but I'd hate to see the situation where there where the game jumped from Normal difficulty to that really insane difficulty without any additional options in between.
Same on the easier end... there should be a few options stepping towards easier rather than just dropping down from Normal to "Narrative" (and just as much as I think there should be a truly insane difficulty level, I think there should be also the option for a combat-less one).
Different people feel challenged at differing levels, so the more increments we're able to select from the more likely we'll all be able to find a level of difficulty we really enjoy. My preference, therefore, would be as broad a range of difficulty as feasible with as many incremental "stops" along that line of progression as practical. If you're wanting the game to "creep" towards being more difficult overall (insanity harder than before and normal harder than before and casual harder than before) - No, I'm not in favor of that. As Sartoz posted above... players who want things to be more difficult for themselves can always just not use all their character's level ups or just not upgrade or mod their weapons.

ME2 had 3 steps about "Normal", while ME3 had only two. I felt like this left "Insanity" a little too easy in ME3 - it played more like ME2's hardcore.

I am fine with games being more difficult. But as games get more advanced I'd rather difficulty be handled more along the lines of dropping more enemies into places, giving them better tactical coordination, etc. than just making them soak more damage and inflict more damage.
It really takes me out of a game when I'm supposed to be this amazing combatant and nameless mooks do more damage than I do and have more health than I do.

DAI wasn't that hard even on nightmare. The only problem I had was Haven before Corypheus showed up.
I like to see survival mode. If a squadmate "dies", they stay dead. So it would be possible to have Ryder by her/himself at the end of the game. excellent. The only way to bring the squadmate back is to restart the playthrough
Insanity in ME2 was tough. I liked it. ME1/3 insanity mode was easy.

However one would think they must have had similar stats from ME1 , yet it didn't stop them from making ME2 far better combat wise. So i'll keep up some hope . Also, I'm somewhat surprised by those stats . What were they for the other modes ( casual, normal end hardcore) ? I would have thought people who replayed the game ( for me ME2 had a lot of replay value , as opposed to ME3 which had less ) would have raised the bar at some point. Do players go through missions just as a formality, in a hurry to get to the next cutscene, dialogues and whatever ?

Which I imported to ME2 ... as my experience grew, insanity became the default. Same for ME3 ... insanity became the default ... is it easy? ... yeah, it is (especially after playing ME3MP) ... but after a while, it's just the norm.
But if I only played the game through maybe twice ... not sure I'd ever get to insanity ... at least the ME2 version. So perhaps that skews the data. Wonder what the data looks like for those who play through the SP game 4+ times? I'm guessing a far higher percentage playing on insanity. Which may be why BioWare bothers ... for the loyal fan base that plays the SP game several times.

Factor in that people start doing speed runs (which are generally done on the casual setting) or back off insanity just because they feel that they no longer have anything to prove and just don't want each replay (which they're doing most likely for roleplaying reasons) to take longer than necessary. I fall in that latter category. I've done a lot of playthroughs on insanity and I have no difficulty getting through the entire Trilogy on that level, including all sections of Pinnacle Station and even the Mirror Match in the Citadel DLC. I'll still play on that level some of the time... but I really have nothing to prove and, therefore, also have no problem backing down to normal or even casual just to speed a playthrough along. If they can increase the top-end difficulty without sacrificing mid-range and the easier options... great. If they they have to pull out a mid-range level such that the jump is too extreme... they are just going to discourage people from trying to replay the game at a higher difficulty. Some people like to step up the difficulty in stages... others don't mind jumping right in at the top end. Which brings me to another point... they should ditch the system that prevents a person from playing insanity on their first playthrough.

If a Squadmate falls in combat, I must restart from the most recent save. In the case of Mass Effect which had some relatively short missions, I wouldn't save very often to maintain the difficulty somewhat.

I agree with the notion that MEA Insanity should be at least as difficult as ME2.
Just need some good old fashioned tiers like this:
Casual Normal Veteran Hardcore Insanity
I don't think it matters if only 5% of the players finish an insanity run since the entire point of the top difficulty for any game is for die hards anyway that want to play the game multiple times.
