SMARTCHOKE MOD
I've tested it on the tutorial mission with a human male adept (no weapon bonus whatsoever), with a Scattershot X. I have the Smartchoke X too (and the smartchoke X only -- no other mod). The smartchoke X should give 40% spread reduction. I can confirm this doesn't not work on a Piranha II, a Ruzad X and a Scattershot X. I haven't tested all shotguns. Did the same test with the Turian gold character with the Rank 6 for spread shotgun reduced (-25%) in her passive. No change. It looks like spread reduction doesn't work in general. BTW BW doesn't have innate piercing in MEA. I have clips of everything I mentioned, but I'm too lazy to upload it; you'll have to trust me or test it yourself.

Did the same test with the Turian gold character and her "-25% spread reduction for shotgun pellets" passive. No change. OP updated.
Are there other mods that don't work? We know the penetration mod for Sniper Rifle doesn't work. Maybe the Barrels X don't work too (it only stun what's behind cover, with no damage). That would explain why so many weapons suck damage-wise.
edit: the sniper rifle penetration mod seems to work for some people. It's been unreliable to me at least.

Why does this seem so familiar, like I've seen a bug like this somewhere before.. but I can't put my finger on it... was it maybe another Mass Effect game?
Anyway, thanks OP for this find. There's probably loads of other bugs to discover.
Also, someone please, we have to confirm if Shockwave evo 4B finally works.

I mentionned the receiver for the SRs.
Did some testing:

I have a PAW I that ticks for 4% per shot. Putting a Magazine V on it makes it tick for 3% per shot. So magazine capacity would seem to work on the energy weapons.

I was really afraid of specing 4B for my Human Adept (at first I chose damage/force), but after seeing someone posting a message about a suggested build for the Adept, I decided to try it myselt and (apparently) it is working.
