I thought that I recalled her elaborating that Chicago is her hometown. I had that conversation as Sara Ryder. Maybe a Chicago native can tell us what D46 is? Likely a district and/or neighborhood. Beyond that, I can't say. It's not a ME reference, but a real world reference, from what I can tell.
Edit: A d46 search brings up a consolidated school district in Chicago. Pretty much as I expected. This may be a personal, inside reference, added by a writer.

Some real world tie to whom ever did that part. Remember getting Jack in ME2 off the prison ship. There were three cells numbered with area codes up in Canada.
