I may have to test this later, but I think this is how it works. You cast Invasion on an enemy, and then you drop their health to 15% while the enemy is still affected by Invasion to trigger the explosion.
For example, cast Invasion on an enemy at say 100% health. Then drop their health to 15% or lower, and see if the explosion occurs (assuming that the enemy is still affected by Invasion).

I may have to test this later, but I think this is how it works. Also, from what I've seen it isn't a "boom" explosion but more of a character rag doll effect.

I've noticed this as well; the explosion doesn't always trigger when their health drops (and yes, I'd infect them first and then inflict damage afterwards). Not sure if it's a bug or something else, like the power's description perhaps being incomplete and there being some extra conditions for the explosion bit.
