I think, you worry too much. The people (even a KISA) can make inconsistent decisions, this is one of the reasons, why they sometimes regret their decisions. The humans aren't machines, sometimes their decisions coming from their personal sympathy, or combined their sympathy AND moral preference.
For example, Zevran's case: the Warden can spare him, because does not kill a man who has given himself up, but even not really OOC, if kill him, because s/he see that he could jeopardize his/her cause.

Mainly because I haven't fully figured out his morality yet and have a hard time deciding what I'd like him to be. The general idea is that he is a follower of rules and believer of honor before pragmatism, though he is willing to make an exception if said rules are evil or if sticking to his moral code would result in a greater loss of lives. He also spares morally gray characters and kills full on villains. I had him kill the bandits of Lothering out of sheer disgust. But now I need to stop and think about how that will affect future choices, since I must keep this character consistent. If he kills enemies whenever they seem completely evil, then I'd be obligated to kill almost all of them, as it always beg the question "So you hate those who [insert here] but [insert here] is tolerable?". And if he always kills those who pose a threat to people, this might make him a vigilante type, which is incompatible with the KISA I want him to be. But at the same time, I don't want him to be overly soft. I really can't contribute anything to your quandary, but, I have to say that I greatly respect your dedication to your RPG and hope that you are able to solve your issues.
