In one of the earlier patches, they "fixed higher difficulties at high levels", or so they say.
What exactly did they do? Do enemies still have way too much HP at insanity over level 90? Or is it more streamlined now? I'm doing NG+ at insanity and am 77 now, but kind of afraid to continue levelling... Or shoudln't I be anymore?

The problem with a question like yours is it becomes subjective to how a person views the content combined with their build and skill level.
If you are nervous that it might get harder you could just do what you think is mandatory. In my opinion the primary story mission and probably the ark missions would be the most important. I have been told there are a few side quests that will also impact the final stretch of the game I cannot tell for certain if they do or not or just change dialogue. The majority of the quests aside from some on the Nexus are playable post the end of the main story line.
Edit:
You can always drop the difficulty if you find it gets too tough as well unless you are only doing it for the achievement for beating it on insanity then would I just do more of a hurried play through if you are concerned.

I think they toned down the ridiculous scaling a bit. Regardless - I've started a completely fresh playthrough for insanity - the scaling just isn't fun at high levels. Too much bulletsponge.

I simply stated opposite is true, they're too easy. He more or less asked what the patch changed and if enemy npcs still had the insane amount of HP at higher level. It's got little to do with difficulty - we can agree mobs are easy on insanity, but the time it takes to kill them is just outright tedious, not challenging. What he did not ask, was if you found them easy or not.

I just finished a NG+ Insanity on Sat, after the patch dropped. Enemy types didn't seem too difficult. I definitely didn't notice any scaling issues. Thank you kindly for your answer. I have a strong build, and I'd hate for it to become less powerful as I grow. By the time I got to end game almost every power was fully evolved. Add in lvl 10 armor and wep's, well, I'd hate to imagine a scaling issue that would've been problematic.

Enemies started scaling massively at high levels in NG+, while the player's power reached a bit of a cap at this point. So the game just became progressively harder with few ways for you to catch up.
Unfortunately the nerf made all of Insanity easier at once, which is a bit of a shame as I liked its difficulty. Then again it did make some enemies lose their bullet sponge status which is fine by me.

I saw that. I would prefer smarter tactics, but that would mean smarter companions and sorry... that's not a strong point in Andromeda. It also makes it more of a chore without the power wheel. That alone was jarring for me. Engineers and Infiltrators are much more difficult to master (it is possible, git gud.) but the payoff for insanity is not worth it. In the original trilogy, I could save moments for devastating combos because that "game interruption" mechanic worked as intended. There was no good reason to remove it. It removed my situational awareness and left me watching PeeBee shoot into crates instead of targets. Did the gameplay improve from the original trilogy? Hell yes. Do I miss the power wheel? Hell yes. Did I adjust and embraced what was given? yes. grudgingly.

Honestly insanity shouldn't even be called insanity.
I'm kinda amazed how much to the extent they nerfed the level scaling. I was currently doing a challenge run with just 3 skill point investments and while using just the predator I and avenger I. And the fun part is i'm playing with level 132 scaled enemies. And don't have access to cheese like hold the line, saving barrier or life support.
