I've lost three squad members, while all of them had loyalty.

Picked Legion for the Vents, but he died. Grunt died as a leader of the 2nd firing squad. These two had cut scenes that made sense. I can't say the same for Mordin who randomly flopped dead on the floor during the retreat to the ship. I think I've royally screwed the Krogan since I've asked Mordin to destroy the cure. uh oh... Also didn't send an escort with the crew... it's a good thing I was never nice to that psychologist girl to feed the fish or the fish would cry... well they would have had they not been floating belly up long before. Where is the button for fish feeding anyway?? I've never found it. Gave the collector station to Invisible Dude.

Jack kicked ass as the leader of the 1st squad and as a shield. Brought Garrus and Morrinth to fight a big ugly reaper embryo. Did gewd. Both lived. Game said I could pursue relationships with the team members via continuing post final mission. Total lies. Garrus gave me the same all blowing off the steam line after we've already blown of steam. Finished game at the point.

I thought I had successfully used Legion in vents before, but I usually used Tali.

It is pretty impressive how they tied all that together.

The vent person dies if they aren't legion, tali, or kasumi

if they aren't loyal

OR if you choose a bad fire team leader. Grunt doesn't work as a leader

Oh, right! There was a time limit for Vents! Well, this could be the reason why Legion died during adonniel's playthrough, since Legion was loyal.

Also, adonniel chose Jack as first leader. Choosing the wrong leader can lead to death for the person in the vents so this could be the case as well.
