Like other bugged "stuff" ... reloading will often fix a glitch. Liam's event always worked for me, but lots of other "stuff" will bug out on any given run. When I encountered them ... at first, was very frustrated ... then learned to reload the most recent save and generally they were fixed.
At a couple of points, on a couple of different worlds, reloading will take forever ... you may think the game is locked up ... it's not, something is wrong, but eventually the game will load. Mostly happens if you get killed in specific spots on Eos, Kadara and Elaaden. Once you're past those points, the game will load normally.

IIRC the nav point is slightly wrong; you've gotta go talk to August Bradley, who's stood ~100 metres away. It confused the hell out of me for a few minutes on my most recent playthrough.

Restarted mutiple times but still won't work. Will try speaking to bradley.
I am having same prob with pb remnant scanner in haarval vault. Cannot do this either despite restarting this multiple times.

IIRC you have to be either really close to the navpoint or one/two metres towards the Tempest for that to trigger.
At least that is what i remember from my last playthrough. Bradley was still standing at his usual position then, contrary to what Fluffy experienced...
(and the obvious: You have to have Liam in your party)
But if it doesn't work, you are not missing much.
