DA wiki says she's a "spirit", but that she comes from the forest, not the Fade.
Does anyone know anything more about this? Or have any theories?

Well, we know that some spirits can survive being in the physical world even without a host. Perhaps she was a spirit trapped on this side of the Veil when Solas erected it. Plus due to all the bloodshed in the forest making the Veil there thin she could have enough access to it to survive before being bound to a wolf by Zathrian.

Strange, I guess I never read the wiki, and possibly didn't pay close enough attention in game. I assumed when she said Zathrien bound her to the wolf he had summoned her from the Fade like where most spirits come from.

She is an avatar of the Brecilian Forest itself. There was no individual spirit or "being" that existed prior to Witherfang/The Lady's creation; there was just life without consciousness, which Zathrian gave form and thought. Witherfang represents the ferocity of nature, while she represents its beauty.
The closest comparison I can think of is Ego the Living Planet, which had an avatar in the form of Kurt Russell. The difference, obviously, was that Zathrian’s curse created the Lady, whereas Ego created his own avatar.

She's not really different from any other Spirit. She was an ethereal being that represented an ideal in the Fade. It's just, rather than the ideal being something like "justice" or "desire", in her case it was "the forest". Then she was bound into a "real" form, but since it was one intended to represent her ideal, it didn't twist her into a monster like you see with demons.

As others have said above, the Lady is a type of gestalt entity that compromised all life within the Brecilian Forest and personified every attribute that people had given the forest (beautiful, dangerous, etc). Witherfang existed because Zathrian bound the spirit of the forest within a great wolf. The Lady form and the Witherfang form therefore serve to reflect the duality of those afflicted by the werewolf curse, both man and beast, as well as humane and savage. If/when the curse was ended, the Lady disappeared because there was no reason for either form to exist anymore. Solas claims in Inquisition that "a spirit's natural state is of peaceful, semi-nonexistence", which we see reflected in the Lady's statements about having not possessed any consciousness or form until the curse. (Semi-related, in The Last Court, there's a similar entity in the forests of Serault called the Horned Knight.)

A spirit that took L.A.R.P-ing too far... and succeeded... She found love, joy, sorrow, understood what it meant to be mortal. And unlike Zathrian, she welcomed death, seeing it as just another journey, rather than the end.
