It's bugging me that it's been years since I saw this happen. Clearly there's something in my current playstyle that's causing this scene to never happen, but I have no idea what it is. Maybe I'm skipping the wrong post-mission conversation? Does anyone know what the key ones are?
I just want her to feed the damn fish.

Were you rude or aggressive to her? Were you too formal with her? From what I gathered from the Wiki and my own experience, providing you are friendly and encouraging it will eventually happen. Just make sure you talk with her after every mission. It may be you haven't reached the trigger point yet. I had to lose at least one batch of fish before it happened. It was my first run and I hadn't even realised the fish could die, or Kelly could feed them for me. Then after they did, I checked what I had to do to avoid it, other than rush to feed them at every opportunity. Eventually the option came up for me to invite her to dinner.
The only other thing I can think of is that you have already advanced too far in a romance and this has cut off the Kelly conversation. However, I don't think that is possible because the romances only advance after a certain time and initially it is only a low key dinner date that none of the romances object to.

Let the crew die by not sending a squadmate as escort during the suicide mission will cause Kelly to die, preventing her from feeding your fish. (That's an obvious one.)
As long as you choose all the flirtiest or nicest answers when talking to her (either top-right or top-left), she will eventually agree to feed your fish.
I can't say for sure which dialogue options would lock you out of it. I have never experimented with it. I either try to get her to feed my fish (rare), or choose neutral options/tell her at the very beginning to keep interactions formal. Telling her to keep your interactions formal is probably the soonest you can lock her out of feeding your fish assuming the game does not give you more leeway. I've never tried.
Pretty sure the conversation to feed your fish happens post-Horizon. It happens after you can invite her to dinner (assuming that option was available to you).

As far as i know, the first conversation has to run pretty much as follows: 1) You have to agree to call her Kelly (selecting "The pleasure is mine." then "Sure" then "Let's chat." 2) You have to Investigate everything, so you have to ask her "What do you think of Cerberus?" After she responds, you have to ask her "Does Cerberus hate aliens?" and after she responds again, you need to tell her "You are very loving." 3) Finally, you need to ask her "Are you happy here." When she responds, you need to reassure her that you won't fail by selecting "We won't." After she responds by saying she knew she could trust you to catcher her, you need to select the flirty option and tell her that you might do more than catch her. The trigger phrase for that is "I'd embrace you." After that, you just need to keep talking with her when you make your rounds. When she suggests that she could hug Garrus, you should also ask her for a hug. When Samara is recruited, you need to tell her that she's prettier and she should respond with the thought to setting aside time to get to really know you (dinner invite). When I'm getting her to feed the fish, the invite always seem to come right when I recruite Samara, but I pretty much always get Samara after I get Thane, so I'm not sure if that changes if you recruit Samara before Thane. For me (recruiting Thane first), she does also comment on Thane and her flirting line there will change depending on whether Shepard is paragon or renegade at that point in time; but as I said, no dinner invite at that time.
It's a sequence that generally feels (for me at least) fairly natural with a MaleShep, but a little awkward with a FemShep; but I generally avoid playing as a FemShep because I don't like her flirty tone with Jacob. it's not as bad with Kelly, but still seems awkward to me.

The "you're prettier" line is a lie to me so I never chose that. I'm afraid I've always been frank and disincline to compliment her / smarm up to her so there's no dinner and no fish offers. But then, after the first PT, I didn't buy fish any more and simply use Gibbe for ME3 to get the fish.

The "you're prettier" line is a lie to me so I never chose that. I'm afraid I've always been frank and disincline to compliment her / smarm up to her so there's no dinner and no fish offers. But then, after the first PT, I didn't buy fish any more and simply use Gibbe for ME3 to get the fish. Well, beauty is in the eye of the beholder. Samara (obsessed child hunter) doesn't turn my crank at all. I've only tried to romance her once for the sake of experience. I often pick Morinth and then arrange to have Morinth die as second team leader (vindicating herself by dying to ensure the others made it through).

If you're not friendly with her, why would she want to go to dinner with you? Or feed your fish? The "I" there was me, not Shepard. That Shepard likes her just fine.
