Da4? Even if we somehow keep a hero from dai would we still be the inquisitor especially if it takes places in tevinter?
Any other known organizations that would even include non human races and would include both warrior types and mages

If we play a spy for the Inquisition, we may get a fake name. Otherwise I don't know. The Inquisition was a exception, inquisition are rarely called. And the Grey Wardens are known to be the rare one who don't care about race and mage or not, because the Blight is so bad.
Even small mercenaries band might be picky about races.

DAI didn't really use prisoner as a name/title. For names/title it use Your Worship, Lady/Lord surname, Lady/Lord Herald and Inquisitor.
For the next game, they could just use the preset surname, being the member of an organisation or getting a fancy title isn't exactly a requirement in a story.
It's very doubtful DAI protagonist is coming back for the next game as the protag, or the Inquisition in official capacity, going by what is said at the end of Trespasser.
Your organisation requirements covers some of the mercenary organisations in Thedas and slaves. As well as the Qun, except that one is less nice to mages (but people would be able to get a "mute" protag). We also don't know if the next game will have (all) playable races.
I see "hey you" as the best name/title personally though. It's universal. BioWare should use it more.

They'll probably do that optional first name thing like they did with Scott and Sarah in MEA I like that, it's a neat option. Even though I'm not overly fond of the name they picked most of my Ryders are Sara because I like hearing the first name on occasion.

I don't think the name thing is going to work if we have a race selection, though. It works for ME because we can only choose to play as a human. Granted I've only played the game once and only done one romance, but I can only recall Scott's name being said just a couple of times. While the feature was a fun addition, it wasn't this huge game changer for how scenes played out. If they hadn't had it, the game would hardly be different.

Bob the Qunari, Bob the human, Bob the dwarf and Bob the elf. Tammy-Mae the Qunari, Tammy-Mae the human, Tammy-Mae the dwarf and Tammy-Mae the elf. Sounds legit to me. Could get away with a single name for all species if the protagonist is (capital Q) Qunari. ...although at that point it’s a name AND a title, so I guess that puts us back where we started. (plus they’d have to come up with some way of handling the name for mage players, maybe they’ve successfully hidden the magic until now?)

A surname is most likely, but it depends on what the game will be. I do sort of hope some more difficult topics come back and we get called "knife-ear" or "dog lord" or "maleficar", depending on background and future choices, as well as any title we may acquire along the way.
Earning a title seems to be a thing, though. Warden/Warden Commander are both well-respected titles. As is Champion and the Inquisitor was also "Your Worship" or "Herald". My bet goes that we will be called "Average Joe" or "Billy Noone".

DAO: We started as the Warden and became the Hero of Ferelden DA2: We started as Hawke (surname) and became the Champion of Kirkwall but we already knew from the beginning that is where we were headed. DAI: We started of as the Herald (of Andraste) and became the Inquisitor. These titles seemed interchangeable. However, depending on your race, you were given a surname which was used on occasion, most notably when you are introduced at the Winter Palace.
So aside from our family name in DA2 the PC has always been known by some sort of title and other people, even our companions, seem to refer to us by that. I must admit it always seemed a bit odd to me when my lover is calling me by my title instead of my own name. However, it did bring out how your own identity becomes subsumed by the legend that you become.
So I fully expect them to come up with some sort of title by which you are known in DA4. If your origins are in Tevinter then that will be fairly easy as Dorian says they love their fancy words and titles. If you are working for the Shadow Inquisition then probably you will have a code name, which could be just about anything as long as it was gender neutral.
If they allow different races then like DAI they will probably give you a surname and then people close to you may refer to you by this.

MEA had the option of allowing us the use of a personal name, but even then it was only used a couple of times. Even though I played as default Scott, I didn't notice an appreciable difference.

The Warden also got to be Warden Commander and Commander in DA:A, which is my favourite title so far. Since the Inquisitor will return, I like for him/her to return, see what I did there Bioware, hint, hint, there won't be a need for a new name and/or a title. Well even if the inquisitor does return they're gonna need to be referred to by something else anyway by all but the inner circle. The inquisition is either publically disbanded or no longer in their control, with them running a secret organisation in either case. So they no longer have the title half the the time and are trying to work incognito and thus won't want to be known as the inquisitor by most the other half. Personally, whether it's a new pc or the inquisitor I'd like them to use the last names of each of the races with the occasional sir/mam, ser/serrah, or equivalent, when appropriate. Then have the companions not use the pcs name/title after they get to know you. It should be writable, I don't recall alistair ever calling the HoF "Warden" when you spoke to him, and you don't really refer to people by name in one on one conversations much.

That effectively ensured I would never use the default names. I'm hardly gonna use the name Sara on my first char when I know my seconds gonna have a sister who looks and acts different with the same name, nor use the name Scott for my second having just had a brother of that name on my first. Too mentally offputting for me.

At the rate machine learning is improving, that honestly could happen before DA4 comes out. This post has some samples of the current state-of-the-art, WaveNet: deepmind.com/blog/wavenet-launches-google-assistant/And here’s an older post from the same project, has more voice samples but they’re slightly less polished: deepmind.com/blog/wavenet-generative-model-raw-audio/Google’s currently using it to improve their assistant, but you can imagine how interested filmmakers (or game studios) might be. Initially, people might use it to patch in single words so they don’t have to re-shoot to get a certain tone. IIRC, Disney has already experimented with stuff like that. Then it’ll get realistic enough (with emotion, inflection, etc) to hold up for longer and longer lines. Maybe a late addition to the script can be completely synthesized instead of being reshot. Then there’ll be big-budget movies that are fully CG... audio included. I’m pretty confident that will happen in our lifetimes, maybe even in the next decade or so. Just like with drivers, restaurant cooks, radiologists, and all the other fields that are on the cusp of automation. Paying for an algorithm is much cheaper than paying humans - who need enough money for food and shelter. So there’s a ridiculous amount of money in ML right now. People who develop self-driving cars command salaries double or triple the typical software engineering pay.
