This is a list of Codex entry locations I have discovered while playing, or checked personally after their locations became known to me from other sources. All these entries have been personally re-checked in my recent replay in January 2019, except those conditional on a world-state I did not have. Those available only in pro-Templar games *after* Champions of the Just (which I re-checked in a fork of my replay) have been found in a game in late 2015.

Numbers are dependent on the presence of additional content beyond the main game. The numbers given here assume that all additional content is present. Location descriptions for expansions/DLC:

Frostback Basin: all Jaws of Hakkon entries start with "Frostback Basin".

Winter Palace: "Main Plaza", "Inquisition Quarters", "Baths and Gardens" and "Palace Entrance" are from Trespasser, all others from the main game.

Deep Roads: "Darkspawn Warrens", "Ruins of Heidrun Thaig", "Forgotten Caverns", "Bastion of the Pure" and "Wellspring" are from Descent.

Deep Roads: "Grand Staircase", "Mining and Processing", "Bridge of the Stone", "Hideout" and "Lower Walkways" are from Trespasser.

If anyone has any info on the few missing Codex entries in the main game, I'd be very delighted. For instance, I have never seen "The Tranquil" in the wild, so to speak. I only got it from Willvan's shop in Val Royeaux. Since I got it there, it must be in Haven, Therinfal Redoubt or Redcliffe Castle. Someone said Redcliffe Castle, but I never found it there. It should actually be around Minaeve thematically, but I never found it there either.

I remembered Blackwalls, so thought I might as well find the rest and post them, since I recently finished Trespasser.

Late edit: In Trespasser, in my PT where Blackwall left the inquisition and I never did Cole's personal quest, Blackwall's codex entry was still present at the same spot, but Cole's was not. I will assume it's because Leliana can keep track of everyone except Cole because of his nature. I will also assume then, that the codex entries for other people who can leave will be at the same location as if they where present, though I have not tried it.

Yeah, you don't get the Serault WT ops unless you have completed (I think) The Last Court. It's available to play at DA Keep. I finished it once but got bored (sidetracked?) the second time through.

I don't believe anymore.
