One of the 3 mechanics in this game I hate purely and with passion: effective mouse sensitivity changing at times. Everyone knows it: you're standing back to the wall, you're about to headshot a Marauder but your scope dances like crazy instead and you miss like Beckham in 2004. I somewhat learned when to expect this and be more slow, but it won't hurt to have some solid knowledge about it.
So: did anyone figure out what and when exactly happens and how to mitigate/avoid it? Is it true that disabling Origin In-Game helps? Is there any connection between this and mouse dampening?

Yes. Always disable mouse dampening. ALWAYS. It's garbage and your turn radius is limited when you have it enabled.
Also Marauder head hitbox is bugged (like many other things in ME3) so sometimes even when you obviously hit Mr.Shields in the head, the game thinks you've missed.

I'm thinking about why sometimes mouse acts like crazy even without mouse dampening (I believe this happens). I putted Marauders as an example but it's most noticeable when targeting Nemeses and Phantoms, least with geth.
Or maybe I'm just not used to playing with raw mouse input, or maybe it's the cable.

Sometimes, when my laptop's resources get out of whack (thank you MS Windows) my mouse and the mouse cursor will get flooey.
This isn't when playing games, and so may be unrelated to what you mean, but is it possible some background increase in resource consumption could mess with your mouse while a game is running?

In addition to the damping of the mouse, I recommend that you turn off vertical synchronization because this will reduce the input lag. but I do not recommend disabling frame limit. Also, the game is a bit crooked when you are in the shelter (hard cover), especially when you are sitting. just don't use it during shooting.(meaning if you use fast aiming, when shooting with a sniper rifle. in other cases, it usually does not interfere)
