The Recon Hood bug-eyes glitch has been preserved, though it doesn't happen as often. I have to close out the game in between combat missions to prevent it, though it appears more often in DLC missions (in the 360 days only a console restart would fix it, again in between every mission). I DON'T want to have an emotionally impactful scene ruined by Judge Doom eyes, thank you very much.
I also got my credits and medi-gel wiped to zero when I killed a Banshee but its warp effect killed me just as the autosave activated. Reloading an older save fixed that, but GOOD TO KNOW.

This krogan was walking in the air and got hit by the YMIR's rockets a few times LOL. Helped make the fight go a lot quicker.  And I have a headless experience during Jacob's loyalty quest - from before the most recent patch. Not even sure if the patch fixed this problem yet. |

Not a visual glitch, but sort of funny and sort of not.
So, I'm traveling between two systems in ME2 when my controller runs out of power and a "please reconnect controller" message appears on the screen... only the game does not pause to enable me to insert a new set of batteries. Normandy continues to travel out of control until running out of fuel. EDI intervenes and takes the ship back to a relay system and invokes the "running out of fuel penalty" (1/2 your resources). Shepard continues to ignore her pleas to get fuel and tries to leave the relay system (all again without any input from me as I'm still trying to change the friggin' batteries).
My goodness Bioware... pausing the friggin' game whenever the controller disconnects should be game development 101 stuff!

So this is really kind of hillarious. My wife is playing right now and she took Garrus and Liara to Peak 15...and she gets to Peak 15 and well Liara has entirely dissapeared. She is not in the squad UI thing and also not on the environment at all. This has continued through at least 2 eleavtors and through one cutscene...where Garrus was talking to himself during the whole first cutscene with the Rachni.

Awhile back I had a glitch during the Kasumi's Memories mission in ME2 where I died and the game couldn't find any saves for it, making me have to start the whole mission over again. Reflecting on it, I think what caused it was dying at the same time I blew up the people killing me with a turret. I am guessing this screwed up the auto saves somehow?

Stones floating in the air on Menae (Mass Effect 3: Palaven priority mission)... This glitch comes from original ME3, and it's been brought as is in LE.
(Taken this from a slightly wrong angle; these little stones roughly in the center are actually floating!)

I saved inside Udina's office just after the council meeting in ME3 and I exited the game. Now, when I resumed my playthrough, he's nowhere to be found. I've tried leaving the Embassies and returning and he's still not appearing.

Thankfully I eventually managed to kill him by shooting him in the foot, and didn't need to reload. Yay!
