So far, I've 100% completed ME1 twice on insanity, doing all side missions and completing the Bring Down the Sky DLC both times.
Although I have recceived pop-up notifications for completing BDtS and for Insanity I and Insanity II, and romanced Ashley both times and unlocked the Neural Shock bonus power on my first run, the following badges are grayed out in the Achievements display (despite the fact that some of them were colored after my first run):
Paramour 1 - was colored after the first run and is now grayed out. Paramour II - notification popped on my second run, but the badge is still grayed out
Inanity I - was colored after the first run and is now grayed out Insanity II - notification popped on my second run, but the badge is still grayed out
Neural Shock - was colored after the first run and is now grayed out.
Colonial Savior - Was colored after the first run (after I completed BDtS for the first time), but it is now grayed out.
Despite 2 runs in ME1 and a partial run in ME2 (having done 3 recruitment missions and the full Hahne-Kedar side mission and Zaeed's loyalty mission), I still have not unlocked Recruit (Kill 250 enemies). I've certainly kill more than 250 enemies by now.
The achievements are badly bugged in this game. Please Bioware, FIX this.

The OT has a similar error with the Achievements where they would arbitrarily become grayed out. I can't remember the workaround though. I don't think it was ever actually fixed either.

I'm currently playing on insanity using new game plus. Everything carried over into the new game plus for me. You're not on the Xbox. I believe there are issues with the Xbox launcher that are unique to that platform (see patch notes). I don't know, some of the old issues may have also been unique to the Xbox edition of the game. Things changed a lot when they ported the game onto the PC and PS3.
Another thing that might offset it is if Shepard wears the same armor as Kaidan (if he's the one who dies on Virmire). When going into a NG+, Shepard's armor is duplicated. My Shepard is a soldier, who ended the game wearing Collosus X Heavy armor with 1 Medical Exoskeleton X and 1 Kinetic Buffer VII. His was the only armor equipped that way. In the NG+, he is wearing it at the start, but there is also now another copy of it in his inventory. Kaidan, OTOH, was wearing Collosus X Light armor with 2 Medical Exoskeletons. Liara was also wearing the same armor. At the start of thiis NG+ game, there is only 1 copy of the the Collosus X Light armor in the inventory. When I put it on Kaidan on Eden Prime, there is none left for Liara.
In my previous game, I had different armors for Kaidan and Liara, so it was quite clear with that NG+ game that it was Kaidan's that went missing and not Liara's.

The Xbox Live account shows the achievements as unlocked yet the badges shown under the Achievements menu (under Extras in ME1) show them as not being unlocked. In addition, Neural Shock (aka Medicine) is now not available as a bonus power for new ME1 characters... So, the badge being grayed out now is affecting the game itself. Who knows, it may be connected to the problems people are having with bonus powers not unlocking in ME2.

Every time I quit out of ME1 (Restart Xbox, back to homescreen, back to Legendary screen) my three Paramours, three Kill counts, Melee kills, and Terra Nova achievements grey out in the in-game tracker, only coming back after a romance scene or killing an enemy w/melee respectively. Long service doesn't pop regardless.

Long service doesn't pop regardless. I tried that a couple of days ago (doing another romance), and it didn't bring mine back at all. I've also tried doing 25 more neural shock kills with Kaidan and that didn't revive that one either. I've uninstalled the game and am about 75% done the first 21.58 gigabyte download... so I started up ME1 again, but the achievements I listed above are still grayed out. I've also lost access to Inferno Grenades in ME2 (which I had unlocked earlier), but so far, Flashbang Grenades are still there. I'm still not at Horizon in my first ME2 run, so I don't have any other loyalty missions done.
There are several bug reports about this on Answers HQ regarding problems with the achievements on the Xbox and a post from EA that they are looking into it. I think all that can be done is wait and hope for another patch... and that they actually fix it this time.

The photo mode problem, I think, has to do with compatibility with the Xbox One controller. There was an issue originally with the wireless headset feature than has, they claim, been fixed... but my issues with the D-Pad and photo mode continue.
There were also old bugs in the Xbox Edition of the game (which was originally exclusive) that may have been corrected in the PC Edition when they did the port over... that were never corrected on the Xbox Edition itself (and probably forgotten). Most of those are listed in the Wiki... and I'm finding out, are still there. Those, for the most part, I could probably live with... after all, I've been living with them for 9 to 14 years now. I'm just surprised that they didn't find and fix at least some of them while doing the remaster... even the basic ones... like the discrepancy between the credits in the mission reports vs. the credits actually added to the player's inventory.

Considering how the 360 was the developer fan favorite for being easier to develop for I am surprised that there are so many bugs with the Xbox version of it. Even more so since Xbox really popularized the whole achviement thing on console. I wonder if it is the whole weird hybrid of UE3 and UE4 that they have for the remaster that is causing these technical problems. Or at least causing them to be more difficult to fix.

Just finished downloading a 300+ MB update on the Xbox One. Achievements are now displaying correctly and Medicine (the bonus power associated with Neural Shock) is now listed as available when I start a new character. Looks like the really did fix this particular issue.

Figures they'd patch it while I was away from the PS4. Any other fixes? My understanding from the posts made by the EA rep on Answers HQ is that this patch was to be just for the Xbox version. He didn't indicate that it would fix any other issues outside the one regarding achievements. Prior to the patch, they were unlocking on the Xbox Live account, but not inside the game itself. Since Xbox Live recorded them as having been completed, there was no way redoing sections of the games would unlock them "again" within the game. That is, I could have repeated ME1 a zillion times on Insanity and never "re-unlocked" Insanity I or Paramour I after the previous update had greyed them out.
The update was over 300 MB though... so there may have been other fixes. I haven't detected any yet. I'd be surprised though if this update even downloads on the PS systems... unless the EA rep was not really understanding the nature of this patch when he posted about it.
ETA: I should also dd that Stasis in ME2 has not unlocked at all with this patch. Since it isn't unlocking on all platforms, I suspect they eliminated it as a bonus power completely. That is, it may not be a bug, but a design decision regarding the game's "balance" similar to the decision to put DLC weapons, etc. in the stores.
