I just encountered a bug that has never happened to me before... doing MELE3's Thessia mission on Xbox One, Liara was on the left of my power wheel and Javik on the right (which also means that orders to position Liara would be on the left side of the D-Pad and Javik's would be on the right... except, right now, Liara repositions whenever I tell Javik to go anywhere. In addition, Javik does nothing regardless... that is, he does not reposition at all ever. However, selecting powers from the power wheel seems to work OK.
I'm reloading the game corrected it, but I have no idea what triggered the bug in the first place. Any ideas?

Not just ME3 … have that issue in ME1 … which probably means it's possible in ME2, though I've not seen it yet.
On PC, so issue is cross-platform/cross game. Other than reloading (which also fixes squadmates not sticking with you), no ideas/solutions.

I have had the issue in ME1 of squad mates not sticking with you, but only in the Tunnels segment of Noveria. A long time ago, I did have one squad mate fail to "show up" inside the mako (that is, I only had one showing up in the lower left-hand corner. I think they might have magically reappeared after I exited the mako though (but I can't be sure since it was so long ago).
