What are the crucial moments that determine the Inquisitor's faithfulness flag?

I'm currently trying to decide whether to play an Inquisitor who goes from atheist to believer, or possibly Andrastian but still refusing to accept the Herald of Andraste title.

Since both approaches are less straight forward, I need to be very careful with the dialogue. Can anyone explain in what moments this is decided and what dialogue options may lead to each path?
