As for the headless Nemesis, she was definitely killed by a headshot, I have no idea how she didn't die. Also you can notice that she still could shield gate me.

Seriously, the other day i had a game with an invicible Collector captain. It became invincible with a failed over cover grab (grabbed by me, while someone was shooting him with a Jav). Apparently it was not killed and the game put him in a sort of limbo state with him alive, but he could not get damage, like he was out of the game. Maybe the same thing happened with the Nemesis?

You need a power with DoT, that's the only way those invincible enemies will die, and God help you if it's a shielded enemy while you're a Biotic.
