The reason we don't know what happened to the other "twin" is because they are not really twins. The reason they look alike is because halfway though the game, you are given the option in the Nexus Med Bay to have a sex change.
This allows you to romance squadmates who are not immediately available to you due to their orientation.
It also allows you to edit your face if you're not happy with it, which is a bonus benefit.
The reason why Sara's face is devoid of expression is due to all of the botox which went into ensuring facial hair no longer grows out of it.
And Bioware reaffirms their reputation as a progressive development studio, because transsexual romances matter just as much as gay ones.
