Or do i have to pick a few Paragon options to avoid looking like a battle damaged Terminator?
I didnt realise you could get the scars in ME3

I think there is an upgrade to get rid of them, but if you're importing from ME2, gathering the platinum beforehand to get rid of them will spare you the trouble.

Yes, later on in the game when you find one of the Intel packets, you have two choices, one of which is remove scars permanently.

In ME2, the player needed 50 000 platimnum. I might be wrong, then head down to the medbay to remove the scars. After the coup, talk with Chakwas. She will mention about getting some supplies from a turian named Tactus on the docks. Once done, you will get the opportunity to get rid of you scars masseffect.wikia.com/wiki/Citadel:_Medical_Supplies

The first conversation with the doctor aboard the ship in ME3 will tell you clearly. If, after examining, the doctor tells you to keep up with a positive attitude, you scars will return if you go renegade. If the doctors mentions a "synthetic overlay," you've done the procedure to remove them in ME2 and they will not return regardless of your P/R status.

So, once you look like a terminator, is there no way to get rid of them apart from picking Paragon choices from now on? There is no medical procedure in ME3?

Yeah, I excercised my google-fu and found there is a quest after Cerberus attacks the Citadel, the one with the turian in the holding bay and the medical supplies. This unlocks the option in the Liara terminal. Its funny, ive not even cured the genophage and im in full Terminator mode.
