So...
Originally, it seemed like after each round in multiplayer, my character would get a full health restore.
But then I noticed that my high level character would not get this. Some rounds would get no health restore, some rounds would get partial health restore.
My lower level characters still seem to get full health restore.
Does anyone have enough clues to explain to me how this works? (Or am I bugged and other people always get full health restores?)

I don't get full health restores either. Don't know exactly how it works. I just assume I get around 1/3 HP boost at round end and leave it at that.

I was under the impression it was difficulty specific. Bronze get's full health restore, Silver half of your total health, and Gold for a quarter of your total health.

More precisely, you get a certain % of your total back. 100% on bronze, ~50% on silver, ~30% on gold (based on watching health bar, so take this fwiw.) So, if you avoid damage for a wave or two, you can get back to full health, even on gold.

Balance ... what?

Now that even in Gold your health is restored to 75% after each wave. No it isn't. It depends entirely on the class, as some classes can regen health further than others. As is noted below, you get about 25-30% of your Health back per wave on Gold.
