I have no problems landing headshots on off-host. Is everyone on consoles or something? There is certainly an issue off host. Just try it yourself. Go a quick solo and compare it to being off host. If you cant notice the difference in weak spot hit detection, then i don't know what to tell you.

I don't feel that headshots don't work off-host, I know for a fact they don't (consistently). Maybe the people saying they do work take a few seconds to actually pop off a shot, giving host and client time to sync, but I'm a rapid target rapid fire kind of player, quick scoping as much as possible to maintain good mobility. Headshots are without any doubt whatsoever not working properly and consistently off-host. It's not a problem with my connection either, as I ping no higher than 40ms to anywhere in the continental U.S..

Weak Spot hits are really sketchy off-host.
What I don´t understand is, why the hit registers at all.
IF I score a weak point hit on the target ( clearly indicated by the trail of the shot going through his head ) and he traveled in a slighty bowed path ( including lateral movement ) to my position but his head staying on the same level, the shot should either 1) Miss completely, zipping by his ear or 2) score a weakpoint hit as it still hit his head. Instead it just registers as a normal body hit which is the oddest of possibilities.
If off host playing was all I knew, I would rather say there is a critical hit system in play, giving you a chance to score a weak point hit instead of it being a matter of where you actually hit the enemy. It´s frustrating.

Why we don't get that bug from BF4,when headshots were registered twice.

I've never had this feeling, and im playing with widow or isharay. On host, off host i doesn't feel a difference. Maybe i had luck this far? It's because you have one shot kill weapons. You are missing the weak spot; it's just the pure power of the gun still demolishes them. Try sniping with a semi-automatic sniper. You'll see the difference.

After further testing it out I have "figured out" the problem.
Off host you have to wait, so you cant quick scope or shoot quickly.
If you place your crosshair over the mobs head, or where you know that mobs head will be a couple of seconds from now, you will hit HS.
Obviously, this isnt feasable in a real game. You cant wait a few seconds before taking the shot. So its not lag, because im not playing with 2k ms off host. But you literally have to wait 2 seconds before taking the shot.

I have to end up either shooting above or to the side of certain enemies head. Ridiculous.
