I dont seem to have the ability to unlock the perk for the 4th consumable slot, is this anything to do with the colony choice you make on Eos?
I'm just about to head to Kadara...did I make the wrong choice or will this unlock later?

The requirements for unlocking it are listed in the avp menu. 90% viability on some planet. It won't say it if you did not go to all planets yet. Also avp ends at lvl 20.
