Talking about the conversation with Kallo and Suvi where they tell you that the radiation on Eos has cleared.
During my my first playthroug I went to Voeld first, did everything there, placed an Outpost, killed the Architecht, saved the Moesha. Then I got said conversation, and headed to Eos and was suprised that the Architect there had an intro and everyone was like "wtf is that" even though I've already fought one, but that's is kinda of topic.
On my second PT in NG+ I headed to Havarl first, did everything there, returned to Eos for a quest and was suprised to see that the radiation had cleared already. And after saving the Moesha I still got the conversation with Kallo and Suvi. I figured it was a bug related to NG+ (like the movie night).
Now on my third PT not NG+, Havarl first again, same thing...
This conversation is obviously not supposed to come after saving the Moesha, but rather after helping the Angara either on Havarl or Voeld.

Continuity errors, whether there is a bug, causing conditinal flags not being read, or simply forgotten by the BW team... it is there. And it is, sadly, not just the Architect, or the radiation.
Also, typical for BW games, some characters say their thoughts about a mission once that is done, when you finish like 5 missions before you meet that said NPC, they give you everything. (IE... I never bothered talking to Efvra, but somehow did after Archon was defeated and he made remarks about events that I finished hours ago.)

As for the Eos thing, it seems like it was originally intended that Eos radiation would clear after saving the Moesha but was changed before release, so it would clear after hepling the Angara either on Voeld or Havarl and they just forgot to adjust the trigger of the conversation.

I just thought it was a gate to prevent people from mining one planet to death like in the Hinterlands...
As for the timing of said events, I would guess the goal was to NOT do everything on other planets first, which ruined the order a bit.
