In the Citadel: Family Matters quest, Shepard encounters Rebekah Petrovsky and her brother-in-law, Micheal. Rebekah Petrovosky mentions her husband died several months ago and she's carrying his baby, but shows no signs of being pregnant.

When controlling Joker with the collectors on the ship, instead of following the lighted floor towards the medbay, go to the left and follow the scion. It will not realize Joker is behind it.

After shooting the 3 husks, walk to where they first appeared and look to the right. There is a couple piles of dead bodies. The same with the left side
During the memorial wall scene, the only time Samantha is seen is if she is holding the nameplate or Ashley/Kaidan are holding the nameplate. She will not be seen if the other characters are holding the nameplate.

For those who have played DAO, when looking at the "moon" while in the party camp, it actually is the planet Klendagon that is seen in ME1 and ME2. I looked a few minutes ago and sure enough it is Klendagon

When you first see the conduit, get out of the Mako and walk forward to trigger the cutscene. After gaining control of Shepard he/she will not be able to get back in the Mako to get to the conduit which leads to a mission failure screen. I made a save before doing this just to see what happens.

When talking with Legion, after being activated, he will say "One ship. One will. Many minds. Like the Geth. We study your records. Sovereign told you this on Ilos I didn't know Sovereign said that on Ilos. I wonder if Legion meant to say Virmire instead of Ilos? |

If the broker dlc was completed in ME2 and Javik and James are taken to Sur'Kesh, James will say "what the he** is that?" in reference to seeing the Yahg. Shepard will reply by saying "Ask Liara that sometime".
A few minutes later, Shepard will say "There goes the next Shadow Broker". Vega will reply by saying, "Now that's one mission I wish I could've seen".

There are 4 missiles where James keeps his weights stored that look exactly like the missiles used to take down the reaper in London, except a lot smaller in size. The same missiles are seen in Kasumis' loyalty mission
If you have space hamster, Shepard is able to activate him/her through the wall while in the bathroom and hear the hamster squeak

Before entering the bathroom in Dr. Bryson's office, a painting of a reaper can be seen on the left. Shepard can activate it, if the player chooses
As Shepard fights his/her way to get to Ann Bryson, Shepard will pass 6 paintings of reapers. 1 painting is seen while talking with Ann and another painting can be seen while fighting the uglies before getting to the shuttle

When talking with Victus on Earth, he will have some different lines of dialogue based on what happened during the game. This video has Victus saying Wrex kept his end of the bargain. I guess it would be the same if Wreav is in charge If you don't talk to Victus about his son during the game, his son will be mentioned If the bomb is set off, Victus will mention that Wreav/Wrex didn't send enough. He will say the same if Eve is dead and the bomb mission was completed If the genophage is sabotaged with Wrex in charge, Victus will be angry. He mentions that Wrex just quit and left taking his troops with him or something simiiar |

After killing the brute on Manae, if you start running, Garrus will run faster to stay ahead of you or at least try to. You eventually run past him and get to the point where he can't be seen anymore if you look behind.

During the Rachni mission, when the spore pods are destroyed, no swarmers come out of them, but after fighting the second batch of rachni and uglies, swarmers pop out of the pods. After talking with Dagg/Grunt, the pods are called Gestation pods and swarmers will come out of them when destroyed.

Shepard and edi are the only ones seen getting out of the shuttle on Chronos. The other squadmate is not seen till the player gains control of Shepard
