Hopefully, the combat is nothing like DAI. I would prefer ME3 with about a 20% difficulty increase...through actual intelligent enemy design and not simply adding a layer of armor and shields or more weapon damage. I want it to be tactically more challenging, not just grind through more waves of enemies.

I want it to be tactically more challenging, not just grind through more waves of enemies. Ditto. I'd especially love to see this in MP as well. Loved the challenge of Gold and Platinum. But definitely improved enemy design please. Actually forcing us to change up our gameplay style, or think more tactically how to take down an enemy is more engaging than just having to fire a round of different types to bring down the barrier, then shields, then the squishy beneath the armour.

As a fellow Insanity head, I hope it carries the best traits of all 3 Trilogy games with it into it's hardest difficulty.
ME1 - You were required to hunt loot, sell and buy with some min/maxing to become truly OP, this was fun AF. ME2 - Intense battles like Horizon, The Collector Base and Derelict Reaper were AMAZING, let's re-tread those feels thanks. ME3 - The bump in AI/Abilities and sheer aggressiveness in Insanity sets it apart here, although you become somewhat OP late game.
Give me intense battles that I just manage to get through, aggressively smart AI that flanks you and makes you really think/panic at times and awesome min/maxing that lets me feed my inner Diablo/PoE fiend and we may very well have Insanity perfection.

I was playing the most recent demo for Nioh yesterday and was actually thinking about this very subject.
In that game, when you kill the boss of a mission in the normal game, you unlock the ability to replay that same mission on what the game refers to as "twilight mode" - meaning that in addition to enemies having more health and dealing more damage, it actually changes what enemies spawn. So for example, a section that would normally have five or six soldiers will have a miniboss-type enemy (stronger enemies with more diverse movesets that are generally bigger and more complicated to fight) and two soldiers on twilight. In addition to that, the boss of the mission also behaves more aggressively and gains new attacks. To incentivize playing twilight missions, the loot that the bosses drop is much better.
I think this is the best implementation of a harder difficulty I've come across.
edit: Oh, and as a neat touch, it also changes the time of day to match the twilight name. Completely irrelevant in terms of gameplay, but it's a cool aesthetic that shows an attention to detail.

The first step would be an AI that is smarter than what we saw so far. I recall a scene where enemies armed with assault rifles were slowly walking towards Ryder - instead of running for cover and shooting from there.
Give the enemy snipers and vanguards. Let them use grenades and mines as well as tech and biotic combos of their own. That should keep gameplay lively and challenging without much need for hit point bloat and damage boosts.

And just from currently replaying ME2... could we please get some animations that aren't the waddle, waddle, waddle, with their gun out when not in cover? Like, idk actually have running animations and rolling/jetpacking or what have you for the humanoid enemies. Unless they are in cover, literally all they do is waddle back and forth lol
I'd prefer them to have some actual tactics too.

Not necessarily elevator battles... but I like the idea of hazards and such on the battlefield. Turrets that you can use against the enemy (or hack if you're a tech type Ryder). I'd even like weather hazards. (See something like Gears of War 4 for this... high speed winds can blow debris and cover into the enemy or you)

As long as super easy is still super easy made insanity as hard as you like but do it via actually making that higher levels more difficult not the whole game. I don;t want to be playing a dark soulsesque game where you bang you r head against a boss for 50 deaths until you figure out how to kill him on easy mode.

I doubt BioWare could make a difficulty that people would like since there are many different levels of player skills. Heck I never found any BioWare game on a higher difficulty challenging unless it added a new mechanic such as friendly fire. Personally I like the idea of the Trial System they had in Dragon Age: Inquisition over having a difficulty setting for then it can be customized to making the game how you want it.

On the one hand, I have cerebral palsy. My reaction times are very bad and I have trouble with simple things like holding down multiple buttons at once and tapping buttons rapidly. So I have a vested interest in easy being easy.
On the other hand, I completed ME3 on Insanity, so I'm thinking it should've been harder.

Nightmare in DAI was easy. Insanity in ME3 was a joke--I played Bronze matches that were more challenging. I'd be happy with an Insanity difficulty more akin to ME1/2.

What made Insanity on Mass Effect 1 hard? All it did for me was to make the combat last longer since all enemies had the health and shields of a Geth Armature. I will admit that it was hard at the start if you started at level 1 when you didn't have any gear, but the moment you started to get some reasonable gear it was just like The Division with bullet sponge enemies.

I have Cerebral Palsy which means that I only have one hand on the controls and have difficulty having to use too,many buttons and getting them pressed quickly enough. Although I've gotto admit I'm impreesed by the fact you beat ME3 on Insanity I can only just about do it on Casual and I get my ass kicked if I try to go any higher. That's the main reason why I'm worried about the no pause to aim. In terms of the rest of the game it looks and sounds good but the lack of pause to aim does hav eme slightly worried as to whether I can fire my powers off accurately.

ME3 just started easy and stayed easy the entire game, with the only real exception being the Citadel DLC mirror match(which even then, I turned on all penalties for myself). I actually had to double check to make sure I was even on insanity in ME3.
It'd be nice if insanity could remain challenging throughout the entire game.
I'd also advocate for the opposite: easy should be easy for most people. Those who have no interest in being challenged by the difficulty shouldn't be forced to. I'm not even against ME3's "Story mode" which made combat largely trivial.

It's very simple: Casual/Narrative/Story difficulty should be playable by almost anyone. If you can use a gamepad or keyboard/mouse, you'll be able to experience 98% of the content. Minimal attention to builds is necessary.
Insanity/Prepare to Die should require knowledge of the maps, enemy weaknesses, crafting and other aspects of the game to complete, as well as good reflexes (for shooters).

Yeah well narrative mode I wouldn't asy is required as that pretty much controlled everything even the RPG elements and I like to make some choices for myself so Casual generally worked well enough for me in that I had the ability to choose where the story was headed but without the high levels of difficulty. But yeagh hopefully they can set a level that's good for most of us. I know I'll be playing on Casual but we'll see how it goes from there.

The clone was a step in the right direction since s/he had the same animation and mobility as Shepard.
