DLC in general from any studio. Every software development/IT project team has it's inherent dysfunction. That is just human nature.
But I think Bioware is stuck in a cycle that it needs to break out of. The issues with Bioware appear to be systemic. To change, they need to focus on better design, planning, project managment, and QA. And everyone needs to lose their egos.
