When playing an ME3 default playthrough, Jacob is the only ME2 squadmate, besides Tali and Garrus, that can be at the party in the Citadel dlc.

When facing the first brute on Palaven's moon, Menae, instead of fighting it, turn around and run towards the path you take when searching for Victus. James will say "Commander. We can't let the barricade fall." A moment later the screen says, The Primarch has been killed. After the game reloads, it puts Shepard at that same spot without ever having to fight the brute and you don't get the dialogue from Corinthus telling you where to head to find Victus.

If you don't talk with Chakwas when you see her in the hospital, she will not be there after the coup and never seen again. Her name is not on the memorial wall and she isn't listed as a war asset. You end up without a doctor on the ship.

When facing Gavin Archer during the ex-Cerberus scientists mission, he will kill himself if you don't tell him about David being safe.

In ME1, if you do the Citadel: Family Matter side mission, you will talk with Rebekah Petrovsky and her brother-in-law, Michael Petrovosky about gene therapy. In ME2 on the 26th floor in the Sirta Foundation shop, you will hear them talking and not be able to interact with them. In ME3, Shepard will recognize them on the Presidium if you interact with the couple

In a ME3 default playthrough....
Ken, Gabby, Ruper Gardner, Thane, Wrex, Kelly, Jack and Ashley/Kaidan's names are on the wall. Samara, Zaeed, Grunt, Legion, and Kasumi are not on the wall or mentioned. Mordin, Miranda and Jacob are the only ones from ME2(besides Garrus and Tali) in ME3. Miranda will always die. Jacob can die if you don't rescue the ex-Cerberus scientists and Mordin can die depending on if you cure/sabotage the genophage. Tali will be exiled and not an Admiral
Eve will always die. No peace between Geth and Quarians. The game will assume the collector base has been destroyed, so if you decide to a low ems run and want to pick control, you won't be able to. When talking with TIM on Mars, the collector base is never mentioned.
The Salarian Councilor will always be killed
If you avoid talking with Lt. Kurin on Thessia, you can see an Asari standing on some containers using warp(I believe that's being used) on the husks. You can't kill the husks running to the perimeter. Only the Asari can. You don't have to use the turret to kill the uglies.
To avoid hearing the dialogue about T'soni saying Alliance provide air support, just run past the uglies. If you choose to run past the uglies to get to the barrier for the temple, a harvester will block you. Once you kill it you can activated the lock even if your squadmates are still fighting the uglies.
Before going into the temple, walk to the edge and T'soni will say "There was 5 billion when this war started. I wonder how many Asari have died today?"
If you stay in the same spot and look around, you will not see any reapers at all. You see some damage and smoke, but the reapers are nowhere to be seen. You still can hear fighting, just can't see it.
When I activate all the artifacts, I usually start with the plaque to the left of the beacon. This time I started with the shield and heard dialogue I haven't heard before.
Kaidan: " Doesn't that look like a Prothean?" T'soni: "Not necessarily."
I believe , depending on which artifact is activated first, and not the beacon in the middle, that you get different lines of dialogue.
Before the VI turns into a Prothean form, you can see 3 or 4 harvesters flying in the background.
Before I went in the chapel, I ran all the way back and got the hydra missile launcher to use on Kai Leng. Of course I wasted my time doing that since it had no effect.
On Rannoch while a squadmate is disabling the first AA gun, run past the geth till you get the cutscene with Dorn 'Hazt. Run to the next AA gun, your squadmate is still trying to disable the first one, Shepard and the other squadmate will kill the geth and that's when the other squadmate shows up.
When using the turret in the shuttle, shoot at Koris for a couple lines of dialogue.
After killing the 3 primes, point the weapon at anything but the target and Joker and Shepard have interesting dialogue.
Joker: "Any damage Commander?" Shepard: "Negative. I'll retarget." Joker: "Well, try not to blow up anything important."
After dealing with the Geth and Quarians, you receive and email from Oriana Lawson, Miranda's sister. This happens if Miranda was killed in ME2.
On Sanctuary, the pods that are seen on the walls are call spore pods and the ones on the floor are called gestation pods.
If Miranda is not in ME3, you can choose to let Henry Lawson walk away. This only happens if you don't use the interrupt. Depending on the dialogue chosen, and the interrupt being ignored, both Oriana and Henry can die

When doing an ME3 default playthrough you call Anderson , Admiral, when you see him on the shuttle in London, whereas with an import, Shepard will say, Anderson.
Before firing the cain at the Hades Cannon, you will see a clock tower with the time 1005. In one of the slides in destroy ending, you see a slide with 4 people and a clock tower(not sure if its the same one) with the time of 1005.
If you don't get on the shuttle in London it will blow up giving you a CRITICAL MISSION FAILURE Evac Shuttle has been destroyed. You get some extra dialogue from Coates if you don't get on the shuttle.
When talking with hologram Jacob, he will say yes sir to femshep.
When you ask to speak with Jack, the guy apparently knows which Jack Shepard wants to talk with. I guess there's only one Jack in the galaxy
After Shepard is hit by Harbinger, his/her armor is blown off his/her body, but yet the other bodies in the same area still have their armor on.
Once you gain control of Shepard, after going up the beam, if you look up you can see a body on one of the tubes.
A lot of the bodies look like the mannequins I saw at the department store the other day
When you reaching the chasm Anderson mentions, turn around and look to your left and right and see waterfalls with looks to be blood in them. Someone with a flycam might be able to see how far down the falls go.
Before moving forward when you see Anderson at the panel, turn around and walk back to the door you came through. If you look back, Anderson will not be at the panel. You may have to move to the left to get a better look to see the panel without Anderson being there.

If doing a default ME3 playthrough or import a save from ME2 with Zaeed dead or not recruited, its possible that if you don't move fast enough that Ambassador Din Korlack is killed . If you do, Shepard then knocks out/kills the two bounty hunters
When Shepard is floating up on the platform, you see his/her left hand and feet hanging over the edge. The next moment, Shepard is on all fours. During that time Shepard regained consciousness to avoid loosing his/her left hand and and feet or at least having them badly injured.

You will also hear Ken and Gabby talk about her committing suicide and believe it was caused from being abducted by the collectors. Wow that's surprisingly.... dark.

However, it recquires a lot of things (really, a lot) in order to achive the war effort: 1) First of all, Conrad has to be alive in ME3. That happens if you speak with him three times in ME1 until you can use paragon or renegade options, and if you don't send him to hell because you have bigger problems than help him in ME2. 2) You must have completed the sidequest of Gavin Hossle (from Feros) en ME1.3) You must have at least 10 Matriach Dilinaga texts in ME1. 4) You must have bought Elkoss Combine license. 5) (plus) If you want Conrad to survive in ME3, you must complete Rita's sister sidequest in ME1.

Most of the time I just talk with him in ME1 and do the mission related to Jenna to have them in ME3. I don't bother with the Hossle guy and the asari writings

Depending on your playthrough in ME2, Jack and Jacob can be the only ME2 squadmates alive at the end of ME3. That's if you encounter every ME2 squadmate in ME3
If you do Grissom Academy, Jack will live whether she's loyal or not. She will be killed on Chronos as a phantom if you don't do Grissom
The only way for Jacob to die is not to do the ex-Cerberus scientists mission. His name will appear on the wall after Chronos. He will survive whether he's loyal or not when you do the mission
Mordin will die/killed depending on curing/sabotaging the genophage. He doesn't have to be loyal
Grunt will always die without being loyal.
Miranda will die if not loyal and can die being loyal depending on Shepard's action's
Samara will shoot herself, loyal or not, unless Shepard stops her with an interrupt
Tali will commit suicide, loyal or not, if the Geth are chosen and can die on the beam run Garrus can die, loyal or not, on beam run Legion will always die
Zaeed and Kasumi will die if both aren't loyal
Thane will always die whether he's loyal or not.

When fighting the cannibals on Earth at the beginning while waiting for the Normandy, if you take the fight to them, after a few moments Joker will say, The calvary has arrived bombing that area with Anderson and Shepard in the blast zone.
Before climbing down the ladder to the building you see the kid enter, turn around and walk to the edge, you will see two skycars below in the parking lot
When the hologram edi takes over platform eva and becomes platform edi, it will have no damage on it from being on fire and the shots that were fired at it from Ashley/Kaidan and Shepard. Does it have its own repair protocol like the Colossus on Haelstom? If so, why does it need med-gel if it "dies" during a mission?

If you don't talk to Vega at all, he will not be seen in the mess hall cooking eggs.
If you want to save lives while running to the beam, turn around and run backwards towards the beam. You see no one except your squadmates and only see about 2 maybe 3 gunships fly over.
If you don't do the Turian Platoon mission before Thessia, it will be removed from the galaxy map after Thessia is completed. And if you talk with Primarch Victus in London he will not mention his son, but does mention that Wreav wasn't able to get more clan support. I would guess he would say Wrex's name if Wreav is not in charge.

When you enter the building on Sanctuary, the window on the left has a spot that can be activated with the squadmates having a couple lines of dialogue.
After healing Wilson in ME2, he says to use overload to explode the cannisters. The cannisters explode before he uses overload and a moment later you see him use the power.

When doing Kasumi's loyalty mission, someone will mention that Vido Santiago was attacked by Zaeed Massini and thankfully was able to escape. Of course to get this dialogue, Zaeed's mission has to be done first and the workers are saved. Depending on when Shepard recruits Garrus, Archangel will be mentioned
In Hock's vault, he has 6 statues of Krogan. A statue of an Ogre from Dragon Age. Two miniature replicas of a mass relay. A statue of the Inusannon, the species before the Protheans. The statue can also be seen on Ilos in ME1.

After killing the first Scion that is encounter, during the IFF mission, if Garrus, Tali or Jacob are on the mission, they have dialogue talking about the dragon's teeth that is seen in the background. To have this happen, walk towards the dragon's teeth where a prompt is seen for the player to activate.
When opening Shepard's private terminal with team status highlighted, there are 4 silhouttes. Garrus, Thane, Tali and Mordin

If you wait to go in Bailey's office before Chronos, he and Khalisah will have the same conversation as when you first see them near the beginning of the game. When talking with her before using the elevator, the dialogue and interrupts are the same.

When talking with Adams about the GX12 thermal pipe, Ken will mention that a crew member was killed, if the shield upgrade wasn't purchased, but if you do get the upgrade after the suicide mission, Ken will not mention that a crew member died

If you wait to do the Turian Platoon mission after finishing Rannoch, Tali has some interesting dialogue during the mission along with Ashley. Then bring Tali and Ashley on the bomb mission for some dialogue they share.
Back on the ship I got different dialogue from Hackett and after talking with Ashley at the memorial wall, and thanks to lakus, go to engineering and hear Tali and Ashley talk over the intercom. You will also hear Tali mention the Collector base mission. Its the only time when Ashley is on the ship that she speaks with another character besides Shepard.

When fighting Kai Leng on Thessia, if you run past him the barrier for the chapel is up and when he recharges, the gunship won't fire on you. At least it didn't fire on me
If you speak with Samara on the Citadel after doing the Monestary mission and before fighting the reaper on Rannoch, she will mention the reaper on Tuchanka and if you talk with her after Rannoch, she will mention the reaper on Rannoch being the third one that Shepard has defeated.
When going back to the Citadel, after curing/sabotaging the genophage, Shepard walks to the cockpit wearing his/her casual clothing and when Joker checks emergency channels, Shepard's weapon appears out of nowhere on his/her back or on the hip if using a pistol.

This dialogue can be heard between two Salarians on Sur'Kesh
Salarian soldier: Agree with them or not, our orders are clear: ensure the smooth handover of the Krogan detainees. Salarian soldier: Who gave the order? Salarian soldier: Our superiors. That's all that matters. Salarian soldier: But they're going to cure the genophage! Everything we've done to stop the Krogan will be for nothing. Salarian soldier: When I want your opinion, I'll promote you. Until then, stay sharp and keep your mouth shut.
