That sounds like an issue with the game file. Try clearing the cash and or uninstalling the game. I did uninstall everything, shut down the Xbox (didn't unplug it though - forgot; but I was told a full shut down clears the cache anyways). I'm in the process of reinstalling (for 12 hours so far due to my slow internet). Reading things at Bethseda.net, I'm not the only one who experienced a problem leaving the vault though and it seems that part of the issue may be connected to a Bethseda update of some kind... not that I recall an update being downloaded recently, but the Xbox One does such things rather automatically, so I may just not have noticed such a message while playing a different game. I honestly have NEVER had such extensive save-breaking problems with any of the ME games or with the Dragon Age Origins game I've started on my Xbox 360. I've had to restart a new character in Fallout 4 three or four times already due to encountering bugs that, if not exactly making it impossible to continue, they made me so leery of continuing that I started over again. At this rate, I'm never going to beat this game anyways... which makes me wonder why I should even bother trying again. Only time I’ve ever had saves break on Bethesda games was because of mods. It very well could be an issue with the files not downloading correctly. That can happen on s slow connection.

I am still on my first playthru, my Ryder has just hit level 32 and the stats say I only completed 35pct of the game, lol. Just finished building an outpost on Kaadara. Great characters on that planet, namely Sloan vs Reyes Vidal...the voice actors for each of those characters did an amazing job.

I really liked the sudoku puzzles. I wonder what percentage of players did and didn't. I also have to wonder why have those mini puzzles in the first place in an action game when there are bound to be a lot of people annoyed by them. I enjoy solving them. What I don't understand is the people complaining about them. They gave people a perfect way to bypass them... just use a decryption key. If they run out of those or are running low, they can purchase them quite cheaply from any vendor. There are also a total of 22 sudoku puzzles scattered throughout the entire 100+ hour game and you can carry up to 9 decryption keys at a time. It's hardly a burden on anyone. They put them in for those people who do like a game to give them some breaks from action and combat without forcing the players who want straight action to play them. I think they did it well. It's just another example of how people like to bitch about Bioware for no good reason.
Meanwhile, I'm now playing Fallout 4 (on console, NO mods) and my power armor just disappeared before my very eyes when I climbed into it. Before I got in, it was visible on the rack; after I got in, it was gone and not in the rack inventory or the workshop inventory... just gone. I'm left standing there in the rack only scratching my head as to what happened. I tried 3 earlier saves (one all the way back to just after Concord) and the same thing happened. That save file is toast. Over 50 hours in and the game is wasted. I even restarted a new game... got to the end of the vault sequence, activated the vault door... and the door did not open. That character has no way to exit the vault... even after reloading a save I made just before activating the vault door. When I played Oblivion, I encountered a problem with items disappearing from containers inside bought houses (my house in Bravil to be specific). I hadn't played the game for about a month and when I came back to do so, everything in my house had randomly despawned/reset. Their games have an issue with not remembering all the items the player collects/uses. They try to cover it up with the 'NPC's can steal" feature they dump in, but that is clearly not the reason for my items disappearing the way they have... in two different Bethseda games and on two different consoles (Oblivion was on an Xbox 360 and Fallout 4 was on an Xbox One).
The bugs in ME:A are nothing compared to the bugs I see in other games by other developers. Bioware rules. I'm going back to play ME:A again. I'm doing a lot of reading over at Bethseda.net and I still haven't found anyone who has had this issue. Closest is someone indicating that pieces of their world builds have been disappearing. I did find a resolution to the second issue I had on my restart save with the vault not opening and am confident that downloading the Creation Club stuff more slowly will probably solve that when I start over again (yeah - I'm over my initial butt hurt and am willing to try again one more time). My internet has been so flakey today though that I'm still redownloading the last of the GOTY package. Getting a clean download of this game has been a real challenge. The six DLCs that come with this edition also must be downloaded online using a code and that is what I've been doing all day now between internet brown outs that stop the installations. Last DLC is should be done about an hour from now if all goes well... at 1.47 Mb/s. Last time, I started playing before it all downloaded. This time I've decided to wait it out until it's done. Then I'll pick and choose more carefully about which CC items I download and also not download them all before exiting the vault. Of course, that doesn't stop the game from dropping offline and then coming back online repeatedly as I'm playing. Hopefully, that isn't what is messing up the file saves.

Achievements are disabled when mods are used (that never bothered me though). If you're doing Fallout 4 for the achievements then don't use it.

If you get fed up with the games guns, there's tons of gun mods to choose from (there's one that overhauls the mini gun, making it more useful and deadlier in game).

I opted to allow points spent into the weapon choice of my "class" for free, letting me pick my "bonus power" freely without being limited in choice due to point constraints. Since I'm playing as a Sentinel (Leader training), I use pistols only, but I'm going for Combat Fitness as my "bonus power" for the extra health/shields, regeneration (4), extra ammo (5), and the low-health damage resist (6). Because I have enough points in Pistols, I can invest in Combat Fitness. I'm dying a lot less. But it's certainly a different challenge. I'm very accustomed to switching to rifles and backing up or switching to shotguns for added punch and force, but since I am limiting myself to pistols I have to stay close or I waste ammo. My weight capacity is still low, so I can only carry one pistol (for now) without incurring recharge penalties. Overall, it feels like a new game. I still have my max-level Ryder, and when I switch to that file I feel super-powered, what with a variety of high-caliber guns, top-notch armor, and flawless skill synergy. Overall, I like my "self-imposed challenge". It forces me to play differently, and I have a lot more respect for classes that "only" use guns. As a powers-focused support, I need teammates with high firepower (Cora, Vetra, Drack). It's tough, but I'll make it.

If you have the resources and schematics, make yourself a Hornet with Beam Emitter, Autofire and a bioconverter. Its very powerful and you use health to refill the clip and as a Sentinel I use Inferno to prime and Energy Drain to detonate combos and energy drain tops up your shields and maybe health. If you have the Fusion Mod of Adrenaline on your armour it also means you can use your powers more often without a cool down penalty.

I have been using MEA Fixpack for age but can't even remember what it does!!!! These are the Mods I use, most are cosmetic: Are you only using Pistols? I built a pretty powerful Carnifex (again thanks to Element Zero) that used Burst Fire which was pretty fun.

I probably don't belong here, but I wanted to chime in anyway. Andromeda is sitting on my shelf, but I haven't fired it up yet. With some initial issues clouding the release, I delayed picking it up. I also went back and forth over trying to determine how to make my canon fit with my overall list of customs.
I told myself I have to play a couple of older games in my backlog before I am allowed to start Andromeda though.

I told myself I have to play a couple of older games in my backlog before I am allowed to start Andromeda though. The game received 10 patches after release so a lot of the issues it had were fixed. Is it a perfect game, no but it is one of the best games I have played and having played MET it ranks only behind ME3 for me. Does it still have issues, yes but most are minor. Not sure what canon (from MET I am assuming) you are trying to fit with your list of customs but really it is a stand alone game that incorporates MET history and technology etc. Give it a go and give it a chance. I hope you'll be pleasantly surprised but I am biased I am on my 14th Play through of the game.

I used your list to check out what would I need I'm using PAW, Hornet, Hurricane and Rozerad + Remnant Cryo Gauntlet. All guns are beam except hurricane throws crapton of sticky grenades Do you put Kinetic Coils on your Cryo Gauntlet? KC's on your melee weapon also increase the damage on your other weapons. Another weapon EZ suggested that I made is the Hesh with Auto Fire and using seeking plasma +Kinetic Coils. It does so much damage but it is REALLY loud. I have to shut the door of the room that my PC is in.

I know the thread has been quiet for a while, but I just thought I would chime in to say that I started my first playthrough. I have fine-tuned my single-profile build, so no more respecs in the med-bay.
I also want to say that I have been pleasantly surprised. I have enjoyed a number of the story quests, I like most of the characters, and the combat is entertaining for my build. Waiting until the end of the patch cycle probably helped considerably.
Still not a big fan of the massive areas, but I have been only stopping to perform menial tasks so long as I'm driving by. I have attempted to cut out some extraneous bits, and I never go out of my way for fetch quests. Despite trying to avoid some content, my game completion is only listed as about 51% with about 65 hours, but maybe that percentile will go up rapidly soon. I put the "Hunting the Archon" quest on hold for a while (grabbed the transponder, but avoided Gil) and played around with some vaults and Loyalty Missions. Either way, I will definitely not play any additional characters outside of my canon this time.

Mods is a godsend. I'm on my third playthrough. Game is so nice on a new pc which can actually handle it. It really sucked how PS4 runs the game with medium mesh settings and low-end DoF. Looks beautifully cinematic with the higher sampled DoF like Inquisition or Witcher 2 did. I'm trying to be less horny this time. Fun to see where dialogues go if you don't flirt right away
