Pardon my french, but I just ran into the one of the nastiest fucking bugs I ever have encountered for Mass Effect 1. I hope no one else runs into this and I want to bring this crap to light.
PS3, PSN. This has also happened on the XBox 360, according to my research:
Soo...I just finished ME1 with a new character. When I finished with my first character, everything imported to ME2 the way it should have. I tend to shutdown my console after I play ME1 before playing ME2 due to "fun" memory leak issues. Last time, it wasn't a problem. This time, my latest Shep's data wasn't found. After some intense googling, I found I could reload my last autosave and replay through the Saren fight.
From there, what I should have been able to do is: Beat the game, start a new one with the existing ID. Make a hard save, then fire up ME2. The data should be there. I killed Saren again and attempted to start a new game on ME1 with the Shep I just finished. I also skipped the credits as well. The data is there and I hit start a new game with it.
The game freezes and the console locks up. I'm worried now that I have a corrupted save. I had just made a hard save prior to Shep and CO putting on their mag-boots. Hoping that I don't have to do that one again, I fired up ME2 and looked again for Alex. Thankfully, beating Saren and going through the end again fixed the issue.
Very nasty. I thought I wouldn't be able to import my latest Shep. I would have been pretty pissed off if that had happened. I know it's not considered a huge deal, since Genesis exists. It's the principle of the matter that would have irked me.

If you can on the PS Systems (I play on Xbox), go into the ME2 Importer folder on your console hard drive. You will probably see and import save file with the name of your Shepard (not the importer itself). Delete it... it's stuck and clogging up the importer. You should then be able to go into ME2 and restart the import. On the Xbox, it seems to happen occasionally if you start an import and then don't let it run right through to a point where you can save (i.e. after Shepard wakes up). Sorry if this isn't written in great "computerese" - I'm not really a tech-type person.

If the player saves and exits the game after having done 2 out of the 3 facilities and while still inside that second facility, Admiral Kohoku's body will appear in that facility when the player reloads the game. DO NOT interact with that Admiral Kohoku. If you just ignore him and just go onto the third facility, he will appear in that third facility and you'll be able to finish the mission properly.

It's alright, I understood exactly what you meant. I wasn't able to get to that folder on the PS3. It's very possible that I'm overlooking something there. After replaying the Saren fight, I was able to get my current Shep imported with no issues. Started up ME2 last night.

@upupaway It's alright, I understood exactly what you meant. I wasn't able to get to that folder on the PS3. It's very possible that I'm overlooking something there. After replaying the Saren fight, I was able to get my current Shep imported with no issues. Started up ME2 last night. Glad you got beyond it despite not being able to get to the Import folder. On the Xbox 360, one can get to it by going to the Xbox Home Page = Settings = System = Storage = Hard Drive (if that's where the Importer is installed) = Games and Apps = Mass Effect 2 Importer (folder) = Then you should see a file with the name of your Shepard. It says it's a saved game, but it seems to lock up the importer if you exited that game before the manual save is enabled. After the manual save feature is enabled, that saved game file moves from the Importer folder into the Mass Effecct 2 main folder. The first time it happened to me, I was frustrated by it for about 3 days... trying to import different ME1 saves and all of them were crashing (game would just go to a black screen when it should have first turned over control of Shepard before the SR-1 goes down). When I finally deleted the file from the Importer, I was able to import without any issues.

Interacting with Kahoku again yielded the same squadmate laments as before, but no additional XP reward or journal update. I was able to complete Hades' Dogs and got my morality reward telling the Shadow Broker agent to get lost. In a separate test I completely ignored the third bunker and again had no trouble finishing Hades' Dogs. So it appears this was mostly fixed for the PC; the leftover glitch now provides a way to finish the assignment faster, if you want that.

I have to admit, I honestly did not check my journal to see if Hades Dogs appeared after doing only two facilities and interacting with that Kohoku corpse. I went onto the third facility but was unable to interact with the Kohoku corpse there. I was confused at that point and did not think to just check my journal for Hades Dogs even at that point. I reloaded the save I had made inside the second facility, ignored the corpse that appeared, and was able to interact with it in the third facility and finish the mission normally. I do believe though that Kaidan did actually say we had "one more stop to make" but then changed his tune (or perhaps it was the other squadmate that said the OG it's Kohoku" line) after I interacted with the corpse in the third facility.

It's like Gourmetrix said about it being a tutorial issue. It can only be defeated if I have Kaidan throw it. Until that time, it's invincible. That's really weird then because, at least on the Xbox 360, that part of the tutorial is about tossing grenades, not about throwing at all... so it must be a bug or that the PC tutorial is very different than the Xbox. On the Xbox, the area where the player is instructed to use Kaidan's throw occurs earlier, when the team heads up the hill towards the camp and there are 3 husks. The first one is generally very difficult to kill on any difficulty unless Kaidan's throw is used. The other two can be killed in any fashion the player desires. I'm certain this is still the case - I reset my tutorial messages when I ran through it earlier today just to be sure. The player is even given High Explosive grenades by Powell (if they know his name)... which sets the player up for throwing grenades at the destroyer. Extremely sorry for the very late reply on this particular topic, but I ran through Eden Prime a few days ago with Tutorials turned on; I turned tutorials on and reset the tutorial messages so I could run through Eden Prime clean as a 'new' player. I had the same experience as UpUpAway on the Xbox 360. When Tutorials are on, the prompts for throwing a grenade or using Kaidan's Throw are much earlier in Eden Prime than the Destroyer on the train. That Destroyer never had a white health bar. On Xbox, you can turn tutorials off. Under Options > Tutorials, you can turn them off. Is that option available on PC? You may want to turn those off if you keep getting them, especially if you are used to playing ME1 by now.

On Xbox, you can turn tutorials off. Under Options > Tutorials, you can turn them off. Is that option available on PC? You may want to turn those off if you keep getting them, especially if you are used to playing ME1 by now. On the PC you can disable tutorials by going to Options -> Settings -> Tutorials -> Enable Tutorials, then selecting "No." When you do this the destroyer's health bar remains red, and instead of a guaranteed insta-kill, the effectiveness of Kaidan's throw against the destroyer depends on your combat difficulty.

Another fairly nasty bug. I was doing Thane's loyalty mission, and I was at the part where I was tracking the Turian. I was doing fine and got up to him in the club for the last update. Well, all of a sudden the cutscene kicks in while Thane is still talking to Shep. It showed Koliat, firing at the krogan bodyguard and towards the turian. I was completelyconfused.
Apparently it bypassed the little scene with the two workers. Then I get Thane's loyalty mission failure message from him ("Entropy always wins.") Since I didn't hard save during the quest, I had to redo it. It worked fine the second time around though.

dmc1001 I think I know what triggered it. Rather than coming in from the left to give my update to Thane, I went to the right to be closer to the door. Either I hit the door on accident or I went too far to the right. Regardless, that's when I failed the mission. On a hunch, I decided to stick to the left of the turian when I gave my final update. Everything worked fine after that. So yeah...just do your update to the left side of the turian, not the right. It's easy to avoid, but still a pain in the keister.

While this may not be a bug, it is a nasty thing that bears warning, I think. It happened a while ago but once when I was purchasing a crate with credits, I disconnected and when I re-connected, I found that not only had I not recieved the crate but the credits were gone which was the worse of it since it was one of the very expensive crates meaning I lost a lot of credits.
I don't know if this is also something that can happen if you happen to be purchasing a crate when a group splits.

Some weird glitches happening in an ME1 PT on a PC. MEUITM is installed but I've gone through ME1 twice prior with it installed and no problems.
1. On the Citadel, I was in the Upper Wards and looked out a window (the spot where the Terra Firma Party will later appear). The screen went really dark. I could just barely see anything but the game otherwise played just fine. Saving, shutting the game down and restarting it fixed it.
2. I lost control of Shepard at the embassies near the rapid transit. It was like a third person view but there was no sign of Shepard, only the two squad members. Again, a save and restart of the game fixed the glitch.

ME3 (Xbox 360, installed from disc) - Recently, I've been running into a lot of instances where my squad seems to be "stuck" inactive behind cover. They won't move, they won't shoot, and they won't use their powers even when ordered to do so. Playing on insanity difficulty, NG+ game. It's just happened to me three times in a row trying to get through the mission on Mars. (and I have no points assigned to Ashley's Marksman.)

ME2, PC: After I stop Thane's son, I get the mission finished screen, a loading movie is set up, then fades out, but no game is loading, the screen just stays black. Anyone else had this problem and knows how to fix it? I'm using SweetFX and faster loading screen mods.

It's more funny than annoying really. No idea if he's unkillable or I just kept missing.

As far as I know, he just can't be killed after he pulls his Lazarus act... sort of "sign" of things to come for Shepard perhaps. I've also had it where my squad just wouldn't stop firing at him... I got almost all the way to Benezia using the back route before they decided to teleport to me so I could continue the mission.

ME3, Xbox 360, Citadel DLC (downloaded from Xbox Live Marketplace and installed on hard drive) - When starting up a game of Shattered Eezo, the screen shows a view looking between Shepard's legs such that one cannot see the robots throwing their punches. The problem will sometimes repeat through several reloads of the game save and then suddenly will load correctly.

ME3, Xbox 360, Citadel DLC - Unusual Scores Challenge, Armax Arena. I've been trying to trigger the Fatal Error Unusual Scores match. I have received the email asking me to fight with all the modifiers turned on and have tried numerous times both with a +50% from modifiers (i.e. all modifiers) and at +60% (all modifiers but the one that increases player damage to enemies). Each time, I wind up fighting a normal match with all those modifiers working rather than triggering the special one. I've tried reloading the save several times. Fighting matches in between with various combos of modifiers, enemies and arenas and then setting all the modifiers again. Still no luck in triggering the special match to complete the challenge.
I've done this match lots of times, so I'm pretty it's not something I'm doing wrong... but any advice would be appreciated. I'd like to get the full Armax set done with this particular Shepard if I can.

I may be misunderstanding you post, but the modifier (all positive ones) fight does not trigger the 'Fatal Error' fight. Instead you get the mail with instructions for that after the fight.
If you meant that you do not get that mail afterwards, I'm lost too. You could try leaving the strip and comming back. (IIRC that helped me with an encounter that just wouldn't happen, once.)
