So it is not just me who has this problem, so it is a freak problem with no know reason. I have googling around for a bit this evening, and from what I found so far it seems to come down to either accept that I have lost those achievements for my current male Shepard forever ... or use the console some how to get them back. ( At least those that I lost due to this freak bug. )
But that is a no go for you and others playing on Xbox, and that sucks even more.

I have googling around for a bit this evening, and from what I found so far it seems to come down to either accept that I have lost those achievements for my current male Shepard forever ... or use the console some how to get them back. ( At least those that I lost due to this freak bug. ) But that is a no go for you and others playing on Xbox, and that sucks even more. I just got my Flare and Lash bonus powers back... had to redo the Omega DLC from start to finish. Used an old file save to start at the beginning of it and just played through again until the notification popped again. Then I went and started a new Shepard and was able to again see Flare and Lash on the bonus power list. Hopefully, they don't drop off again. I'm getting a little worried about the Unwavering Achievement... still seems to be stuck at 4/27 missions. Hopefully, something will start to tick over again when I get a little further into this current playthrough (which I'm also doing on insanity) - I'm currrently just on Mars.

A bit odd but far from game breaking.
During the Cerberus Coup I'm used to my Shepards running around C-Sec HQ (and why is Bailey's office in the human embassy as opposed to C-Sec HQ? That's another question entirely...)with a permanent shield boost in effect, well the graphics of the glowy blue hexagons superimposed on his/her armour anyway, not sure about the actual effect when you're shot at. This seems to get stuck in place until I've finished the whole mission, so until i've dealt with Udina. However this time Infiltrator Shep had invisible guns in addition to this. Apart from after fiddling around with the mods on the worktable at C-Sec (and then it stuck again the first time Shepard came out of cloak), Shep's guns remained cloaked even when he wasn't. Both weapons, i could swap between pistol and sniper rifle and they'd both be invisible.
It hasn't happened since.
Shep also died without obvious cause on the Tuchanka Brute n Reaper gauntlet. He was in cover behind a fairly large bit of masonry so it wasn't the Reaper, and there didn't seem to be a brute involved. I assume that either a brute spawned on top of Shep or the Reaper shot the cover up.

Assuming you did, did you have to depart the system and return? Did the game show you the undocking animation as you departed? (Sorry, I'm thinking like a software tester; the Land button should be missing if the game thinks you are already docked, so I'd be curious if that triggered other "I'm already docked" behavior.)

Another ME1 run, another bug. PS3, PSN: This one isn't major, but annoying. I first NG+ my previous Shepard (hey, gotta get in the ME spirit and I'm waiting a bit to pick up ME:A,) but had to immediately save and exit while still on the Normandy. This is before you go to Eden Prime. When I fired up the game, the music had cut out. The rest of the sound was okay, but the music was gone. No music on the ship, no music during the close up of the Reaper in the planet message (right before the end of the scene.) This continued all the way to Eden Prime. Frustrated, I started over. Tried with a brand new character, saving and exiting on the Normandy before Eden Prime. I figured it could be some weird NG+ bug, since I've never NG+ in the MET. Once again, it happened. After about 2 hours of experimenting, I figured out not to save/exit before Eden Prime. Once I landed on the planet, I did a save then exited. I shut off the console and left it alone for about 10 minutes. I fired it back up and it worked just fine. I'm mentioning this because I scoured the net and I couldn't find anyone that had run into that.

Not sure if this is a bug or not, but I am curious none the less. The past two days I hat moments where enemies who where on a elevated attack position and where hit but not dead would stay in cover? No matter how long I waited they would simply not pop up again? Even if I kept Shepard and the squad members in a open place they would do nothing. I tried using Pull / Slam and the M-77 Missile launcher, but I or my squad members could not kill them either? ( Despite the fact that those enemies hat half or less then that in health left?

Mass Effect 3, Xbox One (backwards compatibility), Original disks, insanity difficulty
This is the first time I've ever encounter this bug in more than 30 playthroughs. When trying to complete the Citadel: Chemical Treatment side quest, Dr. Silon is responding as though I have already given him the treatment (which I did collect on Cyone). That is, he is thanking Shepard. However, the task is not showing as complete in the journal. After several tries, I even went back to an earlier save and redid the Cyone mission without any luck. The task simply will not log as complete even though the doc is behaving like I have completed it.

Tried launching from origin, tried launching from the desktop, tried running as administrator, tried running in windows xp sp3 compatibility mode. Nada. Updated to the lastest driver via GeForce Experience. Updated Corsair Link (doubt that matters).
So... I'm out of ideas.
Any more PC savy people out there that could lend a hand? It'd be greatly appreciated lol
Was hoping to start a new playthrough =/ (why did I leave consoles?? sigh lmao)

No third party program anymore, just Microsoft Security Essentials. I have it set to check for updates and scan every day.
I also gave up on Zone Alarm's free firewall since it kept reducing functionality unless I updated to the paid version. I use Windows Firewall now.
I'm on Win 7. Don't know what the options might be for Win 10.

Thanks! May I ask what you use instead? Hi SGTreed24. I use a PC with Win 10 and the built-in Windows Defender AV. Have never had a problem running any of the software, or software installations, even with the sw I carried over from my Win 7-based PC, using the same CPU and Mobo I've been using for the past 7 years. Used to use AVG for many many years, as it was recommended by the techs at my local PC repair shop. Finally had enough of AVG's odd effects on ME-Trilology and preventing sw installations; setting AVG to ignore certain apps was a pain, imo. AVG prevented the installation of my HP printer drivers, so had to disable it; much frustration until I finally thought of disabling AVG; then the HP drivers installed flawlessly. I've never seen Win 10 Win Defender cause this issue. Win 10 and Win Defender are flawless for my needs. EDIT: I removed my add'l comments here because they were PC-related and off-topic. Always have a little fun each day; among the best advice I was given by my dad. K

Soo, those have been a pain for me sometimes and I found a fix for every single one of them. They're all for PC.
ME1:
To fix missing lava textures on Therum (Taken from a post that I saved. Credit goes to whoever found this out!) ---Update--- It appears this this is now fixed for fresh downloads of the game. It looks like they may removed the cached shader files mentioned below from the master download list.
However, it's possible the problem may still be encountered after installing the Bring Down the Sky DLC. Same solution applies. ---
This affects everyone with the Steam version of the game, so if a mod could sticky this so we stop getting posts about it I'd be thankful.
If, when you land on the planet Therum, you're seeing a bunch of black spaces, there is any easy task you can do to fix this.
Cause of Problem: Mass Effect compiles and caches shaders into special files as needed while the game is running. The first patch for the game included some shader changes that necessitated the shaders being recompiled for the Therum textures to show up correctly. To recompile the shaders, you simply delete the cache files. For whatever reason (probably just a simple accident) when you download the Steam version of the game it already includes cached shader files built on the pre-patch version of the game (it shouldn't include them at all).
To Fix: There's an automatic and manual way to correct the problem, they both ultimately have the same result of deleting the cached shader files.
To Automatically Delete the Files On Windows XP: Assuming you have Steam installed to the default directory, navigate to folder: C:\Program Files\Steam\steamapps\common\mass effect\Binaries\ and run MassEffectConfig.exe. On the left hand sidebar, select the Repair option. Click the Delete Local Shader Cache Files button. That's it, you can close the config program and start the game normally.
On Windows Vista: For some reason the Repair option is not available by default in Vista. Navigate to the same folder listed above, and right click on MassEffectConfig.exe and select properties. Go the Compatbility tab activate the checkbox to run in compatiblity mode for Windows XP. Select OK and double-click the EXE as normal. Select Repair and Delete Local Shader Cache Files as above.
To Manually Delete the Files The file you need to delete is in two different locations. Delete the file named LocalShaderCache-PC-D3D-SM3.upk in the following directories:
<your user directory>\My Documents\BioWare\Mass Effect\Published\CookedPC\
C:\Program Files\Steam\steamapps\common\mass effect\BioGame\CookedPC\

If Jarrahe Station quest keep crashing for you, be sure to have updated your game to the latest version
After playing the Arrival DLC, the Mass Relays cutscenes are muted. Just reload the game to fix it.

ME3: There is a ladder on the Leviathan DLC that Shepard can't climb down. Putting the game on a 800x600 resolution fixes that.

Not sure if this is a bug or not, but I am curious none the less. The past two days I hat moments where enemies who where on a elevated attack position and where hit but not dead would stay in cover? No matter how long I waited they would simply not pop up again? Even if I kept Shepard and the squad members in a open place they would do nothing. I tried using Pull / Slam and the M-77 Missile launcher, but I or my squad members could not kill them either? ( Despite the fact that those enemies hat half or less then that in health left? Late reply, but maybe someone else also ran into this problem. It may be a pathing issue (as in enemies keep blocking themselves on walls or cover and try to shoot through them. This may explain why AoE powers arent' working, like Pull). Loading an autosave is a good option here. But since you've said that this has been for the past two days, usually this happens when your crosshair(or your companion's for that matter) are aiming at the enemy. So, s/he sticks to cover until it's all clear. That's all I can think of, anyways. But try aiming Pull with the right angles,if your Shepard has the ability. In ME3, Concussive Shot also travels the same way as pull. But, if you don't have it as a power, only your companions, you can't shoot them like that and you may need to go with the M-77 Missile Launcher, or, since that doesn't work for you, the Cain.

Well it only happened a few times again after that, and when it did I indeed reloaded the latest quick save. Also I had not thought of my companions being a influence in a case like this. Should ... or better yet once it happens again I will see if I can get a better angle and place my companions where they have no line of sight. Thank you MegaIllusiveMan.
