ME3, Xbox 360, Citadel DLC - Unusual Scores Challenge, Armax Arena. I've been trying to trigger the Fatal Error Unusual Scores match. I have received the email asking me to fight with all the modifiers turned on and have tried numerous times both with a +50% from modifiers (i.e. all modifiers) and at +60% (all modifiers but the one that increases player damage to enemies). Each time, I wind up fighting a normal match with all those modifiers working rather than triggering the special one. I've tried reloading the save several times. Fighting matches in between with various combos of modifiers, enemies and arenas and then setting all the modifiers again. Still no luck in triggering the special match to complete the challenge. I've done this match lots of times, so I'm pretty it's not something I'm doing wrong... but any advice would be appreciated. I'd like to get the full Armax set done with this particular Shepard if I can. You need all negative modifiers to (not the positive one) to beat the second Unusual Scores challenge. To unlock the third and final Unusual Scores challenge (the 'Fatal Error' challenge), you need: 1. Break 9999 in a previous match. (You need this to unlock the first Unusual Scores challenge if I recall.) 2. All modifiers unlocked. (These will trigger the second Unusual Scores challenge). 3. Super Elite enemy difficulty unlocked. (You need this for the first Unusual Scores challenge.) 4. Reapers and Collectors factions unlocked. You do not need the Mirror Match faction. You will fight Collectors, Geth, Reapers, and Cerberus in the 'Fatal Error' challenge. 5. Spin Zone arena unlocked. The 'Fatal Error' match specifically requires this arena. Hope that helps.

Hadn't really had much problem with the Illium ctd corridor bug before, but had a bad time with it recently. The culprit was the specific AA mode I was trying to force, or at least that in combo with other tweaks possibly (maybe altered shadow resolution).

Thanks Red. I finally got the Fatal Error message to trigger. I think I just wasn't saving the changes I made to the modifiers correctly... not hitting the green check mark to confirm them.

Does anyone else have an issue with a particular ladder in the Leviathan DLC? It's on the planet where you rescue that doctor from the huge reapers attack. When I try to slide down one of the ladders, I keep getting stuck, the animation shows Shepard in a position of sliding but he doesn't, then he just stops in the same spot. Eventually I had to reload and try it a few more times before I got down randomly one time.

The ladder is a known bug. What seems to help most is to try and look 'up' immediately upon approach.

In a thread I made today, I got the tip of mentioning a really odd problem I hat with the Firewalker missions. So despite the fact that in the end it worked out some how, I guess it could not hurt to report it in here. I am curious about it. And please forgive me, but I am going to place a link of my thread here instead of explaining it here again. bsn.boards.net/thread/3007/wtf-normal-firewalker-missions?page=1&scrollTo=184925

Mass Effect 3, Xbox One, Installed from Disc; Priority Mars Mission
During the mission, there is supposed to be a Sniper Rifle Extended Barrel mod and a Shotgun High Caliber Barrel mod that you can collect in the one room where the decon sweep is running. There is also supposed to be a Shotgun Shredder mod in the last area before you enter the archives and speak with TIM. I KNOW I collected all of these, along with the SMG Scope, SMG Ultralight Materials, and Sniper Rifle Concentration mod that are the other mods you can collect during this mission. Trouble is, I am now in the shooting range area on the Citadel and I am only getting credit for the last three I mentioned. That is, I cannot equip the SR Extended Barrel although I can equip the Concentration mod nor can I equip the two shotgun mods to my shotgun (this is a soldier build). I also know that I could equip all of these at this point in the game on the Xbox 360. Anyone with any ideas wtf is going on here?

PS3 Oh I am having a bug infested run of ME1. Ash got stuck in an elevator door on the Presidium, that was odd and mildly amusing. But the trouble starts on Feros.
I went to Feros first (suprised at how hard it is to save Jeong, i failed, not that i have any love for him but the flirty comment from Ash afterwards is a treat.) and my team kept getting stuck. They just stand there pointing a gun at thin air where a geth or varren used to be. I've done it twice, once with an Ash/Kaidan combo and once with Garrus and Liara. Ash is bad, Garrus and Liara were both often nowhere to be seen. Luckily they'd teleport in for fights and cutscenes.
The Garrus and Liara knock out the colonists mission ended oddly. Arcelia got stuck in a wall, stopped firing at us but was equally not registered as unconscious, so remained as hostile (she's pretty hostile anyway, I suppose) and it wouldn't let me raise the ship to reveal the secret passage until I bunged a grenade at her. I did find a secret stash of grenades which i'd missed on previous visits though, because otherwise i couldn't physically kill her or knock her out.
And between Zhu's Hope and the ExoGeni tower the Mako seemed to break my game entirely. I'm merrily driving along, trying to kill geth when the Mako just stopped. Nothing worked, i couldn't jump or shoot, i couldn't even get out. I can't remember if i was actively getting fired at but there was not register or damage on my TV screen, no smoke, no fire, no red bits on the Mako icon...nothing. Then I got the "Saren" music and the game just kicked me out, no red screen, not "reload last save" (a bummer, it was ages ago, of course)it just threw me back to the start screen. Reloading had to be from the last hard save, not auto save.
Onwards to Noveria and there's something odd at Peak 15, and it's not just the Rachni. People disappear, when they're talking, sometimes not entirely. Yes, there's the lady on Feros working on the power system who seems to duck whenever Shepard starts taking to her, I assume she's very shy, but then I got to peak 15. I approach the turian guard outside the quarantine lab (yeah, the one who won't die. I met him again later, he and Kaidan embarked in a two way futile firefight for about 5 seconds before I decided to leave them to it.) He won't let me in, maybe it's because Shepard is invisible except for his head and neck! It was both the funniest and most disturbing thing i've seen in ages. Shep, I know you're an Infiltrator but you don't get Tactical Cloak til ME2! The turian scientist who interupts Han Olar in the middle of the same room did it two, one minute he's standing next to the human guy, the minute he talks, bam! he's gone. Peak 15, weird crap.
But I did beat Benezia for the first time without dying, so it's not all bad news.

This glitch affects the squadmate's rally command in 2 unique ways: When not in combat, the squadmates seek their spawn location, not Shepard's location, so unless their spawn location is reachable (i.e. loaded into memory), they just stand there. In combat, squadmates become hyper-aggressive, moving fearlessly into melee range of enemies. This can be a fun way to play through Feros, if you're ready for it. I posted an almost hour-long demonstration of the glitch here. Wow, those are some nasty bugs. I've seen plenty of enemies get stuck outside the playing area but I've never seen non-enemy NPCs disappear like that. I have seen the turian guard at the quarantine lab, who was previously killed, become an enemy. He was still quite adept at hitting Shepard even though he was just lying there dead!

I have seen the turian guard at the quarantine lab, who was previously killed, become an enemy. He was still quite adept at hitting Shepard even though he was just lying there dead! I'm on an Xbox; but I've regularly experienced Arcelia getting stuck in a wall like that for me. Ledra also is one who can get stuck in behind the railing; and Hannah Murai sometimes gets stuck under the freighter. If stuck, they can usually be dropped with an Anti-Thorian grenade lobbed right at the place where they are stuck. (I normally try really hard though to have my Shepard punch everyone unconscious and no use any of the grenades... for the toxic reason Shepard states). Anyway, on rare occasion, that hasn't worked and I've had to restart that area. I now habitually save just before taking the elevator (i.e. just after I've dealt with all the creepers and the first 4 colonists in the garage). The un-dead Turian can usually be handled by just leaving the area entirely (heading back up to elevator or out the maintenance tunnel). Eventually, your squad will usually just teleport in behind you. I've never had the mako freeze up like that nor have I had friendly NPCs disappear or Shepard "cloak" in ME1 though. BTW - Watched your video on the Varren Meat Glitch... great job! LMAO.

So i finshed ME1 with no further wierdness, except one game freeze (The council were calling me a delusional moron prior to lockdown i think. I blamed my old PS3 rather than the software)i had no further problems until... I coudn't reload that Shep into ME2. I know this can be a problem, and I haven't had it before so i turned off and turned on again. I replayed ME1 from the last "resume point" (where i'm, fighting Saren who's now a husk and bouncing around all over the place unless Liara's got him in her biotic mits), was a little more decisive about the human Councillor (picked Anderson, I don't really think that Shep should get the deciding vote but hey) and tried again. Nope, no Sheps to import to ME2. This is annoying not only because I should have a FemShep Vanguard still knocking about (my current is MShep Infiltrator) but it was a near prefect run, i dropped a couple of random civilians and Ethan Jeong, everything else was done. Luckily i'm replaying an old favourite Shep before Andromeda so I could reload a ME2 profile but, annoying.
Any advice? I'm on a Ps3. ME1 runs from the hard disk and ME2 from DVD.

I'm doing at least a brief return to a Shepard I haven't touched in a while. He's on the Mordin recruitment mission. Just got to the point where I found some looters standing over a dead turian. I shot them with a missile launcher. They disappeared. Has this ever happened to anyone else? Can you just shoot them with a weapon and end any conversation with them?

No but i had the return of the disappearing person bug in the same mission.
In the apartment where you meet the human couple who are hiding, they're initially crouching behind a shelf. Mrs Omega was visible throughout the conversation, however Mr Omega (Mr O'Mega? He is now!) kept vanishing from view, he kept talking but he'd just vanish and maybe pop back at the end of the sentence. However, Mrs O'Mega's eyes would drop as if her husband (I assume, I mean he could be her brother) had just fallen to the floor and out of the camera frame. So, it was like he just kept falling over, very odd, I referred them to Mordin, obviously.
I still can't find any of my ME1 files, they're on the playstation but ME2 can't find them. I blame Multiplayer, that's made a couple of odd cosmetic changes to ME3 when i started playing, mostly just the boxes around the pictures of the mods applied to weapons, nothing too horrific. Either that or it was causes by an unsucessful attempt to log onto the network with ME2 to see if i could get Zaeed (no code, 2nd hand game.)
I also have an issue with the furniture in the Citadel DLC apartment returning to default and not being able to change it back between visits, mostly the sofas and armchairs. If somebody comes round and interacts with the soft furnishings, they turn back to the default, i can't turn them back to my choice again and I can't buy them from the shop anymore. Again, not game shattering, just annoying.

My guests keep getting uninvited on the Citadel DLC. I've been inviting people up piecemeal, waiting until I've met with them before sending out the invite. However, the next time I'd go to invite someone it would be reduced to the autoinvited: Traynor, Cortez, James, Liara, Joker and EDI - the people who would be alive in any playthrough. Anyone who might have died on Virmire or the Suicide Mission gets uninvited unless I have the party immediately after the invitations go out.
EDIT: Further result was that when I tried to have Thane's memorial service, I was immediately brought back to Glyph to have the party. Meanwhile, Kolyat, Bailey and the salarian councilor are all by the piano staring at a picture of Thane. They aren't speaking or doing anything else. Tried to post a screenshot but I don't have a site that hosts images and apparently linking to an image stored on Google Drive doesn't work.

There was a thread on the old BSN forum titled "Known bugs in Mass Effect Trilogy" started by Vazgen. Here are the contents of his post, compiled by BSNers. I've included the original links where I could. I had to remove some links that were no longer valid, or if they linked to another thread on the old BSN. I've tried to keep the original formatting as much as possible. Mass Effect 3 Single Player: > Crazy neck twists. Happens to Samantha during non-cinematic conversations if you stand a little behind her. Also happens to Liara. Video. I just noticed that this bug with Liara doesn't happen if the file is not an import from ME2 and no comic was used.

Like, he'll be in the former XO office talking to Liara but have his head turned completely away from her. It's like he can't start to look at her or something. I forget if it only happens at this time but it's very noticeable.

I forget if it only happens at this time but it's very noticeable. I've been tracking this one - In the XO office, it happens 100% when you initiate the question about Glyph (the drone). Then Shepard and Liara look towards the drone, but Shepard's head doesn't come back around to look at Liara for the next lines in the conversation. However, in this most recent playthrough... the only one I've done in ages and ages that isn't an import... I asked the question about the drone (which has a little bit different dialogue from an import) and, guess what, after they looked at the drone, both of them returned head position to look at each other for the rest of the conversation. I've repeated that dialogue a few times now, just to check, and have not been able to trigger that bug. So, as best I can tell, it is definitively connected to the dialogue related to Glyph that comes from an import file... namely, "I see you've brought that drone from the Shadow Broker's ship with you." and Liara's response to that.

Okay I am not certain whether it is a true bug, or a freak of nature glitch screaming "You are punked ". But it is very weird / annoying no matter if it is a bug or a freak glitch. Yesterday all achievements I hat earned so far in ME2 where there ... but I just wanted to play a bit. I remembered that I hat not researched the latest Sniper Rifle Damage I found so I did that. Afterwards I checked my achievements, ( I do not know why, but I did. ) and that was the moment that several achievements that where there yesterday, are suddenly gone. Achievements like the squad recruitments?? Any one else hat this happen, and if so where you some how able to get them back? I tried reloading previous save-games, but that did nothing for the achievements that disappeared. If it helps I play ME2 in Origin.

A couple of odd ME3 bugs I don't think I've seen mentioned. PC version.
Traynor has dialogue when you enter her area. Occasionally the audio clip doesn't play on my rig. I noticed the bug when I had subtitles on.
There seems to be a bug with finishing Kelly Chambers' ME3 storyline locking out the Traynor romance path when you're not romancing Chambers. I don't have a good read on what causes it. If can be cleared up with savegame editing. It's funny because Chambers isn't supposed to lock out any of the romances AFAIK.

I get the Traynor issues as well. As for Kelly, any time I've romanced Kelly (i.e went so far as to sneak some in the middle of the docks holding area - MaleShep), it has locked out all the other romance options; just as locking into a romance option with someone else has locked out the option of sneaking off with Kelly.

- ME3, PC: Matriarch Aethyta will erroneously presume that Liara is Shepard' girlfriend. To rectify that, use gibbed and find plot.int variable 10320, set value to number less than 4. (Or just purely "renegading" Liara helps too, I think) - ME3, PC: Udina and Shepard are both "amnesiac" about the fact that Kaidan (and I presume, Ashley too) already accepted Spectre' offer. Set bool variable 20901 to true. And voilà, "Kaidan/Ashley said you offered him/her Spectre status and that he/she accepted" instead of "unaware" mess. - ME, PC: Liara ninjaromance will continue if you choose "you were wrong" when she talks about mutual attraction. Pick "I just want to be friends".
Conrad Verner "intimidated" bug is hilarious though.

Interesting to know. It was weird when I heard Conrad talk about Shepard shoving a gun in his face when that never happened. I kept imagining he was dramatizing when he took a picture of me in a gun-holding action pose.

ME3, Xbox One, backwards compatible, installed from original Xbox 360 Collectors Edition disk. Omega DLC installed from Xbox Live account. The lash and flare bonus powers, which were unlocked and available in a previous playthrough on the Xbox One, are now no longer available - even to the Shep being re-imported from the previous playthrough on the Xbox One who had flare as his first bonus power and had played through Omega in that first run. (i.e. starting an NG+ game and I can't even keep flare as his bonus power). I'm not sure if it will again become available once I play the Omega again or not... but right now, it's gone mission and I can't see any reason for it. The Omega DLC still loads if I go back to a point in the original save and load a part at a point where he's in the middle of that DLC.
Second bug - Unwavering achievement seems to now be "undone" and "stuck" at 4/27 despite having the "Insanity" achievement (which is the one about playing all the missions on insanity and not changing the difficulty at all after leaving earth). For this one, I suspect some sort of syncing issue since I largely play with the Xbox offline.

Any one have an idea why this happened to my male Shepard? ( Or if anything can be done about it? )

( Or if anything can be done about it? ) We might be in similar territory here even though I'm on an Xbox and you're on PC. I didn't lose all achievements, but clearly for me the "unwavering" achievement shouldn't be locked now and with only 4/27 missions showing when the "insanity" achievement has been unlocked.
